OBJECTIVES:Certain aspects of pulmonary pathology observed in autopsies of HIV/AIDS patients are still unknown. This study considers 250 autopsies of HIV/AIDS patients who died of acute respiratory failure and describes the demographic data, etiology, and histological pulmonary findings of the various pathologies. METHODS:The following data were obtained: age, sex, and major associated diseases (found at the autopsy). Pulmonary histopathology was categorized as: diffuse alveolar damage; pulmonary edema; alveolar hemorrhage; and acute interstitial pneumonia. Odds ratio of the HIV/AIDS-associated diseases developing a specific histopathological pattern was determined by logistic regression. RESULTS: A total of 197 men and 53 women were studied. The mean age was 36 years. Bacterial bronchopneumonia was present in 36% (91 cases) and Pneumocystis jiroveci pneumonia in 27% (68) of patients. Pulmonary histopathology showed acute interstitial pneumonia in 40% (99), diffuse alveolar damage in 36% (89), pulmonary edema in 13% (33), and alveolar hemorrhage in 12% (29) of patients. Multivariate analysis showed a significant and positive association between Pneumocystis jiroveci pneumonia and acute interstitial pneumonia (Odds ratio, 4.51; 95% CI, 2.46 -8.24; p < 0.001), severe sepsis and/or septic shock and diffuse alveolar damage (Odds ratio, 3.60; 95% CI, 1.78 -7.27; p < 0.001), and cytomegalovirus and acute interstitial pneumonia (Odds ratio, 2.22; 95% CI, 1.01 -4.93; p = 0.05). CONCLUSIONS: This report is the first autopsy study to include demographic data, etiologic diagnosis, and respective histopathological findings in patients with HIV/AIDS and acute respiratory failure. Further studies are necessary to elucidate the complete pulmonary physiopathological mechanism involved with each HIV/AIDS-associated disease.

Background: Lungs are a major organ involved in infectious, inflammatory, occupational and neoplastic diseases. Lungs are also involved secondarily in almost all terminal events. Autopsy is an important tool to identify cause and manner of death and hence to establish preventive methods. Aims: To study the histopathological pattern of lungs in 100 post mortem cases. Methods: This was an observational study on 100 post-mortem specimens of lungs received in the department of Pathology, GMC, Patiala for a period of 1 year from August 2014 onwards. A thorough gross examination was done on the received lung specimens and sections were taken from the representative areas. These sections were processed for paraffin blocks; slides were prepared and stained with routine Hematoxylin and Eosin stains. The slides were studied under the microscope and the results were noted. Results: Out of the 100 cases, 63 showed terminal nonspecific changes; 19 cases showed the evidence of chronic venous congestion. 10 cases were of pneumonia, 6 of granulomatous pathology, and 1 case of metastatic deposits of adenocarcinoma. Conclusion: Infections are the most commonly noticed primary lung pathology mainly comprising of pneumonia followed by tuberculosis. Hence, efforts should be made for the prevention of these infectious diseases.

This review aimed to summarize the literature data regarding the pathomorphology of lung lesions in COVID-19 and compare it with lung lesions in bacterial pneumonia and pneumonia caused by influenza virus. The analysis of scientific literature containing studies of domestic and foreign authors of different years related to morphology and anatomical pathology of lung injury was carried out. Special attention was paid to the data devoted to COVID-19 obtained between 2019 and 2021. Based on the study, the main aspects of lung lesions were identified and grouped into blocks depending on the etiology of the process. The review collects and summarizes information on etiology, pathogenesis and stages of disease development, outcomes and morphological picture during the autopsy of patients with bacterial pneumonia, influenza pneumonia and COVID-19 pneumonia. The common features and differences in the course, outcomes and typical morphological findings, most characteristics for each of the diseases were presented in the table. There is a great similarity of morphological findings in influenza pneumonia and COVID-19 pneumonia despite the background of the difference in their epidemiology. Most Russian and foreign authors agree that a key factor in the pathogenesis of the development of COVID-19 is the presence of a specific receptor-mediated pathway of penetration into the cells of the respiratory epithelium. According to most authors, the main morphological difference that determines the severity and unfavorable outcome of COVID-19 is angiopathy and microthrombosis of the pulmonary capillary bed, which aggravate the typical picture of viral pneumonia.
