BackgroundResectable lung adenocarcinoma is dominated by peripheral distribution, and surgical resection is the main treatment protocol. However, high recurrence rate remains after surgery. Lung adenocarcinoma with epidermal growth factor receptor (EGFR) mutation has strong invasion ability, but the effects of this mutation on local invasion in early lung adenocarcinoma have been rarely studied. This study aimed to assess the effects of EGFR mutation on local invasion in resectable lung adenocarcinoma.MethodsA retrospective analysis of 103 patients clinically diagnosed with peripheral lung adenocarcinoma was included. They underwent preoperative bronchoscopy, which indicated grades 2 or 3 bronchial involvement (lumen of the lobe or segment). The associations of EGFR mutation with pleural invasion, endobronchial metastasis, and lymph node metastasis were analyzed according to pathologies of pleural invasion and lymph node metastasis, as well as EGFR gene mutation detected by postoperative pathological specimens. Statistical analyses were performed by unpaired Chi-square test using the SPSS16.0 software.ResultsIn patients with EGFR mutation, pleural invasion, endobronchial metastasis, and lymph node metastasis rates were 62.5, 39.1, and 34.4%, respectively, indicating statistically significant differences (p = 0.003). Meanwhile, the pleural invasion rate in patients with wild-type EGFR was 43.6%, significantly reduced compared with patients with mutated EGFR (62.5%; p = 0.018). In addition, the endobronchial metastasis rate in patients with wild-type EGFR was 17.9%, significantly lower than in patients with EGFR mutation (39.1%; p = 0.005). However, lymph node metastasis rates were similar between EGFR mutated and wild-type patients (34.4 vs 25.6%, respectively, p > 0.05).ConclusionsEarly resectable lung adenocarcinoma patients with EGFR mutation showed a higher rate of local invasion compared with those harboring wild-type EGFR. Background: Lung cancer is the leading cause of cancer deaths and its incidence is rising. The determination of histopathology and stage of primary lung carcinoma is crucial to develop appropriate treatment approach that affects morbidity and mortality. Fiber optic bronchoscope techniques for early detection of lung cancer are a promising tool as they might allow visualizing changes of early lung cancer and also permitting sampling for histological confirmation.Objective: This study was intended to compare the fiber optic bronchoscopy findings and their anatomical locations with the histopathology types in patients with lung cancer.Patient and Methods: A cross section study was conducted during the period from 1st February 2012 to the 31th of July 2013, 49 patients with clinical and radiological findings suggesting primary lung cancer and diagnose later by bronchoscope samples as cases of primary lung cancer chosen for analysis of their bronchoscope findings, at Baghdad teaching hospital / respiratory clinic.Results: Patients with squamous carcinoma (31%) had (67%) visible mass and (13%) presented invisibly as external compression. Patients with adenocarcinoma (41%) had (40%) visible mass and (35%) presented invisibly as external compression while normal bronchoscope was seen in (30%) of those patients so that adenocarcinoma mostly invisible. Patients with small cell carcinoma (28%) had (50%) visible mass and (21%) presented invisibly as external compression.The location of finding in (27%) of the patients with squam. ous carcinoma was in the right main bronchus and (20%) in right upper lobe bronchus while the carina affected in (47%). Patients with adenocarcinoma have vocal cord affected in (25%) and left upper lobebronchus lesion in (25%).Conclusion: There was no relationship between the fiber optic bronchoscope findings and their anatomical locations with the histological types of lung.

Introdução: A necessidade de monitorar o intervalo entre o diagnóstico de uma neoplasia maligna e o início do tratamento oncológico no Sistema Público de Saúde demandou a construção de uma nova ferramenta. Para tanto, foi desenvolvido o PAINEL-Oncologia. Objetivo: Apresentar o processo de elaboração da ferramenta, suas aplicações, potencialidades e limites, apontando ainda perspectivas futuras. Método: O painel foi construído a partir do relacionamento determinístico de informações diagnósticas e de tratamento obtidas no Sistema de Informação Ambulatorial, por meio do Boletim de Produção Ambulatorial Individualizado e da Autorização de Procedimento de Alta Complexidade, no Sistema de Informação Hospitalar e no Sistema de Informações de Câncer, utilizando-se o Cartão Nacional de Saúde e o diagnóstico de neoplasia como chave identificadora do caso. Resultados: A ferramenta disponibiliza três painéis: 1 - Painel: Monitoramento do início do tratamento oncológico, que exclui os cânceres de pele não melanoma e tireoide; 2 - Painel: Casos sem data de diagnóstico por ano de tratamento; e 3 - Painel: Casos diagnosticados de câncer de pele não melanoma e tireoide; e um conjunto de gráficos e tabelas com filtros para seleção. Conclusão: O PAINEL-Oncologia foi disponibilizado aos gestores e a órgãos de controle em 15 de maio de 2019. Trata-se de uma ferramenta ágil e acessível ao gestor para o monitoramento do tempo do primeiro tratamento do câncer. Contudo, as informações apresentadas no PAINEL-Oncologia dependem do registro e da qualidade das informações dos Sistemas de Informação em Saúde dos quais ele consome informações.
