Análise crítica dos escores de avaliação de gravidade da fibrose cística: estado da arte

Abstract: This study carries out a descriptive and comparative analysis of the various types of cystic fibrosis severity scores described in the literature and contextualizes the origin and objective of each. A total of 16 scoring systems were found: 8 are used predominantly for clinical evaluation, 5 for radiographic findings, 2 for tomographic findings and 1 for scintigraphic findings. Despite the criticism and controversy regarding these instruments of assessment, they have contributed to a better understanding of th… Show more

“…Since then, QoL has been assessed in this group of patients, using Disease severity was assessed by Shwachman score: < 40 = severe; 41-55 = moderate; 56-70 = mild; 71-85 = very mild; and 86-100 = extremely mild. (15) For the purposes of analysis, the patients were divided into two groups: those with Shwachman scores of 71-100 and those with Shwachman scores ≤ 70.…”

“…The evaluation of this severity requires the use of objective parameters such as scores, which can be clinical, radiographic, tomographic, echographic or scintigraphic, (19) as well as pulmonary function tests. (20,21) The disease differs from patient to patient, especially regarding the degree of pulmonary and gastrointestinal impairment.…”

“…Each form of evaluation contributes to the evaluation of one dimension of the disease and can be useful in a specific clinical situation. (19) In the present study, three objective measurements were used to evaluate the severity of CF: clinical score, pulmonary function tests and radiographic score. Whereas pulmonary function tests and the radiographic score evaluate only the degree of lung disease, the ShwachmanKulczycki clinical score (13) evaluates four different patient dimensions: general activities of daily living; clinical test findings; nutritional aspects and radiographic aspects of the lung.…”

“…Each criterion is scored from 5 to 25 points, lower scores indicating greater severity. (10) Pulmonary colonization was defined as presenting a positive culture for Pseudomonas aeruginosa, Staphylococcus aureus (oxacillin-sensitive or -resistant) or Burkholderia cepacia.…”
