Alport syndrome: significance of gingival biopsy in the initial diagnosis and periodontal evaluation after renal transplantation

Abstract: Alport Syndrome (AS) is an important hereditary disorder affecting the glomerular basement membrane. Diagnosis of AS is based on the presence of hematuric nephropathy, renal failure, hearing loss, ocular abnormalities and changes in the glomerular basement membrane of the lamina densa. The aims of this case report were to show the changes in the gingival tissues in a patient with AS under therapy with cyclosporin-A after renal transplantation and to discuss the possible role of type IV collagen in gingival bas… Show more

“…Electron microscopy revealed no remarkable thickening and redundancy of the EBM, which were previously detected in a nail-patella syndrome patient (13). Tonofilament accumulation in the basal cells and disrupted and discontinuous lamina densa, which were detected in the gingiva or skin of Alport syndrome, were not observed in our patient skin (14,15). The other complicated structures including the lamina lucida, hemidesmosomes, and anchoring fibrils, were intact (Fig.…”

“…Diet plays an integral role in the maintenance of oral health, dietary modifications due to medical problems such as phenylketonuria can have adverse effects on oral health [ 19 ]. For Alport syndrome, a study showed the significance of gingival biopsy in the initial diagnosis and periodontal evaluation after renal transplantation and changes in the gingival tissues with patients under therapy with cyclosporin-A [ 20 ]. It has been shown for Cystic Fibrosis, that these patients have a high caries risk and that early management is necessary [ 21 ].…”
