Histopathological evaluation of urethroplasty with dorsal buccal mucosa: an experimental study in rabbits

Abstract: Healing of buccal mucosa grafts to reconstruct the urethral plate can be achieved by total integration of the squamous epithelium with the urothelium, maintaining the original histological properties of the graft with no fibrosis or retraction.

“…Our histological results were consistent with the fi ndings of previous studies, where the characteristically intense infl ammatory reaction and infi ltrated polymorphonuclears were gradually reduced into a discrete and chronic response, mainly of lymphocytes (6). These fi ndings show a greater integration of the mucosa and are macroscopically noticeable at the moment of the animals' sacrifi ce.…”

“…Once the urethral plate is removed, most authors justify that surgery should be done in two steps (two-stages). Lately we have carried out clinical and experimental protocols in order to better understand the behavior and healing of the tissue regarding these complex reconstructions, and in this context, we have conducted experimental studies in animal model (6).…”

“…Meatal dilatation was successful in these 2 cases as reflected by spontaneous fistula resolution. The reoperation rate was 31.5% and consisted mostly of simple procedures like fistula closure [3], meatotomy [2], penile curvature release redo [2], and complex diverticula repair in 3 cases. In the 3 cases of urethral diverticula no association between distal stricture and urethral ballooning was found and removal of redundant epithelial tissue was possible preserving the external layers for neourethra cover.…”

“…In 2004, we described a one-stage urethroplasty approach consisting of using a buccal mucosa graft to substitute the urethral plate, allowing for complete resection of chordee and enabling an onlay transverse flap anastomosis covered by a second flap of tunica vaginalis and cremasteric tissue [1,2]. Buccal mucosa is a versatile alternative as the donor site, and proved to show good integration with the adjacent urethra in an experimental model in rabbits [3].…”

“…À la zone de jonction, il existe une barrière nette entre urothélium et épithélium de muqueuse buccale [17]. Il n'y a pas été retrouvé d'inflammation du tissu conjonctif sousjacent contrairement à l'étude animale de Souza et al [18]. L'indication chez l'enfant se pose dans le cadre des sténoses congénitales méatiques ou de perméabilisation incomplète de la membrane urogénitale.…”
