Abstract: The selective retroperitoneal lymphadenectomy constitutes an effective technique with a lower morbidity than unilateral lymphadenectomy, representing an excellent option for the management of patients with high-risk, stage I nonseminomatous testis tumor.

V léčbě testikulárních tumorů hraje zásadní roli multimodalitní přístup, spočívající především v různé kombinaci surveillance, chemo-a radioterapie a retroperitoneální lymfadenektomie. Z důvodu vysoké morbidity a významného pokroku v chemoterapii ztratila v posledních dvou desetiletích retroperitoneální lymfadenektomie svoji původně dominantní roli při léčbě časných stadií nonseminomů. V současné době však díky rozvoji miniinvazivních operačních technik znovu získávají její modifikované a nervy šetřicí formy na popularitě. Tyto typy výkonů vykazují u vhodně indikovaných pacientů onkologické výsledky srovnatelné s klasickou retroperitoneální lymfadenektomií při nízkém množství komplikací. Při léčbě pokročilých stadií testikulárních nádorů hraje stále významnou roli technicky často náročná salvage retroperitoneální lymfadenektomie, při které je provedení nervy šetřicí techniky značně nejisté.
