Laparoscopic radical cystectomy with novel orthotopic neobladder with bilateral isoperistaltic afferent limbs: initial experience

Abstract: Purpose To introduce a new method of constructing an orthotopic ileal neobladder with bilateral isoperistaltic afferent limbs, and to describe its clinical outcomes.Materials and Methods From January 2012 to December 2013, 16 patients underwent a new method of orthotopic ileal neobladder after laparoscopic radical cystectomy for bladder cancer. To construct the neobladder, an ileal segment 60cm long was isolated approximately 25cm proximally to the ileocecum. The proximal 20cm of the ileal segment was divided … Show more

“…In this study, we demonstrated a time efficient neobladder reconstruction and described many surgical improvements which could facilitate the procedure of ICNB without compromising complications. The time efficient neobladder has bilateral isoperistaltic afferent limbs which was reported by us before in the open approach (17). Having bilateral isoperistaltic afferent limbs, the left ureter could be anastomosed with left limb in situ avoiding being exceedingly mobilized.…”

“…Description of LRC procedures has been published previously (6). For male patients, the bladder and prostate were put into a specimen bag and left in the abdominal cavity temporarily.…”

“…For ECUDs, the ileum was extracted through the 10–15-cm incision below the umbilicus. For ileal conduits [7], a 15-cm-long ileal segment was isolated about 20 cm proximally to the ileocaecum; for orthotopic ileal neobladders [8], we choose a 60-cm-long ileal segment. The ileal segments were detubularised along their anti-mesenteric border to form a new reservoir outside the abdominal cavity.…”
