Incidental detection of retroperitoneal lymphangioleiomyomatosis (LAM) - CT and MRI findings with relevance to the urologist

Abstract: A 48 year old lady presented with blunt abdominal trauma following a road traffic accident. She had no hematuria or other injuries. Vitals were normal. Examination was significant for left loin tenderness. Ultrasound was suspicious for retroperitoneal fluid. Computed tomography revealed normal abdominal viscera but multiple diffuse low attenuating cystic retroperitoneal masses showing peripheral enhancement (Figure-1). Lung segments showed multiple thin walled parenchymal cysts bilaterally (Figure-2, arrows). … Show more

“…The CT value of solid tumor components in noncontrast scan is lower than soft tissue density (about 30HU), and the density can be uniform or uneven. On contrast enhanced scanning, the cystic wall was obviously enhanced, but the liquid part was not enhanced [12]. In this case, the tumor size was about 18.8 cm × 12.1 cm × 13.6 cm, and the density was uneven on plain scan, with necrotic cystic changes in the central area.…”

“…The cases of the bladder PEComa are rare and the image data is less. In combination with related literature [11], [12] and performance in this tumor, the authors summarize characteristics of bladder PEComa: CT manifestations of class, oval or irregular shape pouch or solid mass, volume is bigger, often in more than 5 cm, margin out, noncontrast scan presented mixed density, necrosis, cystic change, enhanced scan presented solid tumor component significantly strengthened, and the rich arterial blood supply and venous drainage.…”
