Emergence of VanB phenotype-vanA genotype in vancomycin-resistant enterococci in Brazilian hospital

Abstract: In Brazil, vancomycin-resistant enterococci (VRE) have been reported as nosocomial pathogens since 1998. Recently, in a VRE surveillance in a hospital, we detected two Enterococcus faecalis isolates with vanA genotype and susceptible to teicoplanin. This is the first report of VanB phenotype-vanA genotype enterococci isolated from humans in Brazil.

“…Colonization of patients in hospital settings by resistant enterococci has been associated to the microorganism spread from other patients and/or from the environment as preventive measures to contain their spread may be of little success (1,3,24).…”

“…Bactericidal antimicrobial activity, necessary for the treatment of enterococcal infections, is usually obtained by the synergistic combination of a cell-wall active agent, such as a β-lactams or a glycopeptide, with an aminoglycoside, such as gentamicin or streptomycin (3,25). Several studies have shown increasing resistance of enterococci to antimicrobial agents such as β-lactams, high-level aminoglycosides and more recently to glycopeptides (11,24,25).…”

“…Bacterial strains -Two VRE strains studied, Enterococcus faecalis 28 and Enterococcus faecium 172 were isolated in 1998 during an outbreak (Casa de Saúde Santa Marcelina, São Paulo, Brazil). The other two strains included in this study, E. faecalis 211 and E. faecalis 217 were isolated in 2004 during a VRE surveillance program at the Hospital Paulistano (Zanella et al 2006). The enterococcal strains that are described in this report were isolated from different patients.…”

“…For applications of enterococci in foods, antibiotic resistance is of special concern because genetic determinants of resistance in these bacteria are generally located in conjugative plasmids or transposons (Hasman et al, 2005;Zanella et al, 2006). Antibiotic multi-resistance has been more commonly reported for E. faecalis due to its notorious ability to acquire and transfer antibiotic resistance genes (Citak et al, 2004;McBride et al, 2007).…”
