The recent improvements in the outcomes of patients with acute lymphoblastic leukemia reflect the progress in the field of diagnostics and the achievements in therapeutic interventions. In particular, the availability of more advanced technology in the cytogenetic and molecular laboratories has resulted in the stratification of patients into specific risk groups and thus several novel agents as well as targeted therapies have been introduced. Additionally, precision medicine will be applicable in the near future and thus the recent developments will facilitate the provision of safer modalities of therapy that may ultimately yield not only higher responses but also improved survival rates.This review represents a recent update on the role of various cytogenetic assays and molecular techniques in patients with acute lymphoblastic leukemia. It is composed of a number of sections including: history of cytogenetics, disease etiology and pathophysiology, utilization of various cytogenetic assays diagnostically, disease-specific cytogenetic and molecular abnormalities, common disease genetic subtypes, prognosis and risk stratification, refractory and relapsed disease, novel therapies, precision medicine and drug repurposing.

Neste ano, no Brasil, no Congresso Brasileiro da Sociedade Brasileira de Transplante de Medula Óssea (SBTMO) de Curitiba, a Associação Italo-Brasileira de Hematologia (AIBE) realiza o seu oitavo encontro anual. E, como já é habitual, a AIBE brinda mais uma vez a comunidade hematológica e os participantes do Congresso da SBTMO com mais um suplemento educativo, de atualização, o terceiro na história, sobre temas relevantes na área de onco-hematologia e, em especial, em doenças linfoproliferativas. À guisa de informação e de recordação, a Associação Ítalo-Brasileira de Hematologia (AIBE) surgiu de uma ideia, que de sonho se transformou em realidade, de congregarem italianos e brasileiros em torno da hematologia. Em Genova, Itália, precedido de encontros anteriores, a Associação foi formalizada e, após trâmites burocráticos entre os especialistas de ambos os países, foi definido que os encontros anuais seriam alternados entre estes países, assim como os cargos diretivos. Dentre os aspectos educacionais e cooperativos acordados foi definido que a AIBE teria como órgão oficial de divulgação científica a Revista Brasileira de Hematologia e Hemoterapia (RBHH). Com esta medida, a AIBE passou a ter um peso relevante no sucesso da RBHH, através da inclusão de vários dos seus membros no conselho editorial da revista, produção de manuscritos e de suplementos educativos e atualizados, como o atual, contribuindo ativamente para o progresso e aprimoramento do veículo científico da comunidade hematológica brasileira.Neste suplemento, o segundo do ano da RBHH, mais uma vez a maioria dos temas que serão abordados durante o evento está presente, e as experiências de brasileiros e italianos poderão ser atestadas em suas páginas.Os manuscritos publicados foram elaborados com a característica de minirrevisões atuais, e que abordam temas relevantes da onco-hematologia. Os linfomas de Hodgkin são revistos e, em um dos artigos deste suplemento, Bacigalupo 1 discute os resultados do transplante alogênico mieloablativo, apresentando resultados de estudos em que a alta taxa de mortalidade torna intolerável a indicação do procedimento, sugerindo que este, se ocorrer, a opção deverá ser com regime de condicionamento de intensidade reduzida.Os linfomas não Hodgkin são abordados em vários artigos. Em um deles, de Souza CA e colaboradores 2 apresentam a experiência brasileira do tratamento sequencial de altas doses de quimioterapia seguida do transplante autólogo. BACKGROUND: Despite recent improvements in the treatment of acute lymphoblastic leukemia (ALL), adult patients still have an overall poor outcome. The future of ALL management relies on the introduction of novel targeted therapies. The authors sought to assess if protein kinases (PKs), frequently deregulated in cancer, show an altered expression pattern and can be considered as suitable therapeutic targets in adult ALL. METHODS: The authors studied the PK gene expression profile by oligonucleotide arrays in 133 adult ALL samples at the onset of the disease and subsequently performed in vitro experiments to evaluate the sensitivity to first-and second-generation PK inhibitors of a set of ALL cell lines, as well as of primary ALL cells. RESULTS: The study documents a distinctive PK signature for different adult ALL subgroups; the PKs identified include several tyrosine kinase (TK) genes, especially in E2A/PBXþ Blineage ALL (B-ALL), B-ALL without known molecular abnormalities, and T-lineage ALL. Consistently, cell lines and primary samples belonging to these groups proved susceptible to TK inhibitors. CONCLUSIONS: These results indicate that second-generation TK inhibitors may be effective in ALL subsets other than BCR/ABLþ B-ALL and provide the rationale for testing the impact of the newly developed TK inhibitors in the management of adult ALL patients.
