The desmoplastic limph node reaction as a prognostic factor of cancer of the tongue and floor of the mouth.

Abstract: To determine the prognostic significance of desmoplasia in metastatic lymph nodes of squamous cell carcinoma of the oral tongue and floor of mouth, 37 cases of this disease were studied. Desmoplasia was present in 59.4% of the cases and associated extracapsular spread in 40.5%. The association of desmoplasia with large lymph node involvement occurred in 59.4%. These relationships were statistically significant (p = 0.002 and 0.0069). The results obtained suggest that desmoplasia is a bad prognosis associated f… Show more

“…To the best of our knowledge, no report has considered the possible correlation between CLNM and desmoplastic reaction, but a few have reported that desmoplastic reaction and peritumoral inflammation are significant predictive factors of cervical metastasis [17,43,93]. Gonzá lez-Garcí a et al [17] reported that peritumoral inflammation was statistically significant in relation to CLNM in a retrospective analytic study of 203 patients with SCC of the tongue.…”

“…Therefore, we believe that the fibrous connective tissue represented desmoplasia. Desmoplasia is a host reaction induced by the carcinoma cells invading the parenchyma, and is not seen in non‐invasive or in‐situ carcinoma 2 . Therefore, the findings in our case may indicate extensive intrasinusoidal spread of adenocarcinoma with minimal invasion.…”

“…Sir : When carcinoma cells metastasize to the lymph nodes, they commonly spread into the lymph sinuses in an early phase, and subsequently invade the parenchyma with desmoplastic changes 1,2 . The carcinoma cells remaining within the lymph sinuses may be confused with some benign conditions 3–7 including glandular inclusions, endosalpingiosis, ectopic thyroid, and ectopic salivary gland, that are characterized by the presence of periglandular basement membrane, no destruction of the parenchyma, and/or no desmoplastic reaction 4,5 , 8,9 .…”

“…Tumor mechanical properties and related signaling pathways are emerging as significant factors in HPV − HNSCC. For example, these tumors are more rigid than surrounding normal tissues, and their stromal density correlates with tumor stage, poor prognoses, and reduced survival rates . Furthermore, molecules involved in ECM cross‐linking, mechanotransduction, and invasion also correlate with cancer risk, tumor progression, and poor survival .…”
