-Context -The investigation of risk factors associated with nutritional status could contribute for better knowledge of the malnutrition. Objective -To investigate the incidence of malnutrition and its possible association with many parameters that assess nutritional status and to identify the associated risk factors. Methods -The nutritional status was assessed in 235 hospitalized patients. Malnutrition was defined as present when the patient presented at least two anthropometric criteria below the normal range and habitual energy intake below 75% of the energy requirement (HEI/ER<75%). Gender, age, type of disease, recent weight change and dental problems were investigated as possible associated risk factors. The chi-square and Mann-Whitney tests were used to compare the data and univariate and multiple logistic regressions were used to identify the factors associated with malnutrition. The odds ratio (OR) and confidence interval (CI) of 95% were calculated with the significance level set at 5% (P<0.05). Results -One-fifth (20%) of the patients were malnourished on admission to the hospital and 27.5% reported recent weight loss. Malnutrition (P<0.0001) was greater in patients with malignant diseases. The only variables significantly associated with malnutrition according to univariate logistic regression were recent weight loss (P = 0.0058; OR = 2.909; IC95% = 1.362; 6.212) and malignant disease (P = 0.0001; OR = 3.847; IC95% = 1.948; 7.597). When multiple regression was used in the model which included type of disease, malignant disease was shown to increase the chance of malnutrition fourfold (P = 0.0002; OR = 3.855; IC95% = 1.914; 7.766). When disease was excluded, recent weight loss also increased malnutrition fourfold (P = 0.0012; OR = 3.716; IC95% = 1.677; 8.236). Conclusion -Patients with a history of recent weight loss and those with malignant diseases are more susceptible to malnutrition.

OBJECTIVE: This study verified the association between hand grip strength and body mass index, subjective global assessment and nutritional risk screening 2002. METHODS: This cross-sectional study calculated the body mass index, measured hand grip strength and administered the subjective global assessment and nutritional risk screening 2002 to 118 patients hospitalized at the University Hospital of the Universidade Federal de Santa Catarina, Florianópolis, Brazil. Hand grip strength was compared with the reference values for the Brazilian population according to gender and age. The statistical analyses included the Student's t-test or Mann-Whitney test and multiple linear regression. The results were considered significant when p<0.05. RESULTS: The prevalences of nutritional risk or malnutrition according to body mass index, global subjective assessment, nutritional risk screening 2002 and hand grip strength were 3.5%, 50.9%, 33.9% and 35.6%, respectively. Malnourished individuals according to body mass index had, on average, 11.0kg less hand grip strength than the nourished individuals (p=0.008). There was no association between hand grip strength and the subjective global assessment (malnourished individuals had -2.8kg; p=0.078) and nutritional risk screening 2002 (malnourished individuals had -1.5kg; p=0.352). CONCLUSION: Hand grip strength was associated with body mass index but not with the subjective global assessment or nutritional risk screening 2002.

OBJETIVO: A desnutrição pode afetar adversamente a evolução clínica de pacientes hospitalizados e aumentar o tempo de permanência hospitalar. O objetivo deste estudo foi certificar critérios para avaliação e acompanhamento do estado nutricional de pacientes hospitalizados aceitos pela comunidade de pesquisadores, docentes e profissionais da área de nutrição clínica. MÉTODOS: Com este propósito aplicou-se um questionário via Internet, à comunidade científica, cadastrada na Plataforma Lattes - sistema nacional de cadastro curricular unificado on-line -, para apoiar atividades de fomento de agencias federais e estaduais. O questionário é composto de recomendações acompanhadas por uma escala Likert e espaços para justificativas dos participantes em relação à opção de aceitação parcial ou discordância do procedimento proposto. Foi considerado aprovado o procedimento que teve concordância total ou parcial maior ou igual a 70,0%. RESULTADOS: Responderam ao questionário 35,2% dos profissionais contatados. A maioria (84,0%) era professor universitário, sendo 62,0% de universidade pública; 67,0% eram doutores; 63,0% tinha mais de 20 anos de formado; 74,0% eram nutricionistas e 25,0% médicos. Os procedimentos propostos versam sobre as seguintes necessidades: triagem para definição de complexidade da atenção nutricional, indicadores de avaliação e monitoramento nutricional durante a internação e equipamentos e protocolos para o atendimento nutricional. Todos os procedimentos foram aceitos pelos entrevistados. Foi proposto pelos autores um modelo de triagem para definição da complexidade do atendimento nutricional, considerando os procedimentos aceitos. CONCLUSÃO: A aceitação dos procedimentos propostos fortalece o reconhecimento da necessidade de implantação de padrões para a avaliação e monitoramento nutricional nas instituições hospitalares.
