Genetic effect on apgar score

Abstract: Intraclass correlation coefficients for one- and five-min Apgar scores of 604 twin pairs born at a southeastern Brazilian hospital were calculated, after adjusting these scores for gestational age and sex. The data support a genetic hypothesis only for 1-min Apgar score, probably because it is less affected by the environment than 4 min later, after the newborns have been under the care of a neonatology team. First-born twins exhibited, on average, better clinical conditions than second-born twins. The former … Show more

“…The second twin is at greater risk of lower scores, which can be due to longer delivery time, risk of hypoxia, nondefinable fetal presentation before birth to decide the better tactics or complications during delivery. The proportion of low Apgar score in first-and second born reported by Franchi-Pinto et al (1999) corresponds with our findings. Fetal presentation at birth showed opposite effects in first-and second-born twins and should be further investigated, together with mode of delivery and in application of horizontal/nonhorizontal classification of presentation.…”

“…Our findings on birth order agree with previous findings. The first twin is in better clinical condition (Franchi-Pinto et al, 1999;Haest et al, 2005;Herbst & Kallen, 2008;Kwon et al, 2011;Morley et al, 1989;Thorngren-Jerneck & Herbst, 2001;Usta et al, 2002;Wen et al, 2004). The second twin is at greater risk of lower scores, which can be due to longer delivery time, risk of hypoxia, nondefinable fetal presentation before birth to decide the better tactics or complications during delivery.…”

“…For this reason, statistical tests to demonstrate significance of such large differences were considered superfluous. As noted previously by Franchi-Pinto et al (1999) in newborns who remained alive, singletons have a sig- nificantly smaller proportion of Apgar scores under 7 (1 min and 5 min) than twins. For newborns who died in the first week, there were no significant differences between singletons and twins in the proportion of newborns with Apgar scores under 7 (Table VII).…”
