A randomized, pilot trial comparing full versus escalating dose regimens for the desensitization of AIDS patients allergic to sulfonamides

Abstract: Sulfonamides are drugs extensively used in the management of AIDS patients. However, the use of sulfonamides is often associated with the development of allergic reactions, provoking the substitution of the drug (by another that may be less effective); alternatively attempts are made to desensitize the patient. Objective. Compare two drug regimens (full vs. escalating doses) for the oral desensitization of AIDS patients allergic to sulfonamides. Material and Methods. AIDS patients with previous allergic reacti… Show more

“…Although co-trimoxazole was confirmed to have prophylactic effect against Pneumocystis pneumonia in SLE patients, it often causes adverse events, even at prophylactic doses. Adverse events include life-threatening conditions such as toxic epidermal necrolysis (TEN) and Stevens-Johnson syndrome (SJS), hepatotoxicity, haematological toxicity and gastrointestinal manifestations [4]. …”

“…Due to the poor specificity of such events in transplant patients who are receiving many drugs and are at risk for developing cytopenia or liver and skin manifestations of GvHD, it is likely that TMP-SMX is too often discontinued and replaced with a second-line drug with a lower prophylactic efficacy. Moreover, we do not use TMP-SMX desensitization protocols, mainly because they are not completely safe [14] and their benefit over rechallenge is unclear [15,16]. As expected, no PCP case was observed under TMP-SMX prophylaxis, while 7 (9.6%) cases were observed under ATO.…”

“…Indeed, isosporiasis is characterized by an evolution marked by recurrence over several years, even after initiation of an effective ARV treatment and curative treatment [7]. The latter is currently based on cotrimoxazole [8]. However, its use may be limited in many patients with hypersensitivity to this molecule.…”
