BackgroundIn Brazil, leprosy has been listed among the health priorities since 2006, in a plan known as the “Pact for life” (Pacto pela Vida). It is the sole country on the American continent that has not reached the global goal of disease elimination. Local health systems face many challenges to achieve this global goal. The study aimed to investigate how patients perceive the local health system's performance to eliminate leprosy and whether these perceptions differ in terms of the patients' income.Methodology/Principal FindingsA cross-sectional study was conducted in Londrina, State of Paraná, Brazil. Interviews were performed with the leprosy patients. The local health system was assessed through a structured and adapted tool, considering the domains judged as good quality of health care. The authors used univariate, bivariate and multivariate analyses. One hundred and nineteen patients were recruited for the study, 50.4% (60) of them were male, 54.0% (64) were between 42 and 65 years old and 66.3% (79) had finished elementary school. The results showed that patients used the Primary Health Care service near their place of residence but did not receive the leprosy diagnosis there. Important advances of this health system were verified for the elimination of leprosy, verifying protocols for good care delivery to the leprosy patients, but these services did not develop collective health actions and did not engage the patients' family members and community.Conclusions/SignificanceThe patients' difficulty was observed to have access to the diagnosis and treatment at health services near their homes. Leprosy care is provided at the specialized level, where the patients strongly bond with the teams. The care process is individual, with limited perspectives of integration among the health services for the purpose of case management and social mobilization of the community to the leprosy problem.

Reorientation of the public health policies in Brazil over the last 20 years in association with a stable rate of new-case detection prompted the establishment of a decentralized leprosy control strategy. The aim was to move from a vertical model associated with general dermatological services to one in which the diagnosis and treatment of the disease would be integrated into the primary care level of the national health care facilities. Once patients demand for leprosy reference centers began to be affected by the process of integrating leprosy diagnosis into the basic health care services, it was necessary to determine the profile of all our referrals in light of the new decentralization policy.ObjectiveWe evaluated the profile of patients referred to the Fiocruz Outpatient Clinic, a reference center for the diagnosis and treatment of leprosy in Rio de Janeiro, RJ, and analyzed the origins and outcomes of these referrals.MethodsThis is an observational retrospective study based on information collected from the Leprosy Laboratory database at Fiocruz, Rio de Janeiro, RJ, Brazil. A total of 1,845 suspected leprosy cases examined at the reference center between 2010 and 2014 were included. The originating health service referrals and diagnostic outcomes were analyzed as well as the clinical and epidemiological data of patients diagnosed with leprosy.ResultOur data show that the profile of the patients treated at the Clinic has changed in recent years. There was an increase in both the proportion of patients with other skin diseases and those who had visited only one health service prior to our Clinic. Among the total 1,845 cases analyzed, the outcomes of 1,380 were linked to other diseases and, in 74% of these cases, a biopsy was not necessary to reach a diagnostic conclusion. A decrease in new leprosy case detection among our patients was also observed. Yet, among the leprosy patients, 40% had some degree of disability at diagnosis.ConclusionThe results of the present study demonstrated the importance of referral centers in support of basic health services within the decentralization strategy. But, the success of the program depends on the advent of new developmental tools to augment diagnostic accuracy for leprosy. However, it should be emphasized that for new diagnostic methods to be developed, a greater commitment on the part of the health care system regarding research is urgently needed.

Objetivo: Descrever o perfil clínico e epidemiológico de portadores de hanseníase submetidos ao atendimento na Unidade Municipal de Saúde da Marambaia no município de Belém – PA. Método: Estudo observacional, descritivo e retrospectivo, com variáveis quantitativas. Foram analisados prontuários de pessoas atendidas nos períodos de janeiro de 2007 a dezembro de 2011. As informações foram registradas em ficha própria, abordando dados demográficos e clínicos. Os dados foram analisados por meio do Teste z e teste de Correlação de Pearson. Resultados: A amostra foi de 127 prontuários, obtendo variáveis demográficas como a média de 39,48 anos de idade, predominância do sexo masculino (51,00%), maioria eram pessoas da raça parda (15,75%), e pertencentes à área urbana (97%), com grau de escolaridade de ensino médio completo ou incompleto. As variáveis clínicas encontradas foi o tipo Multibacilar incidente em 54% casos, não demonstrando significância estatística entre os tipos clínicos (p=1.0). Foi identificada a presença de manchas na maioria dos indivíduos (70%), correspondentes a maior parte de 2 a 5 lesões corporais. Não foi observada relação estatisticamente significante quanto ao grau de incapacidade x forma clínica (p= 0.5). Houve o registro de 77 casos com grau 0 de comprometimento. Conclusão: Identificou-se um perfil de indivíduos do sexo masculino, com média de idade de 39,48 anos, pardos, provenientes de área urbana. A maioria apresentou-se como caso novo e com a forma multibacilar dimorfa, apresentando a mancha como o principal tipo de lesão.
