Objective: To evaluate in vitro the endogenous pH, the total titratable acidity (TTA) and o Brix of pediatric medicines. Method: The sample consisted of 22 pediatric medicines belonging to five classes: antibiotics, antihistamines, anthelmintics, non-steroid antiinflammatory drugs and corticoids. The pH was measured with a potentiometer, total acidity was obtained by the AOAC method and o Brix was determined by refractometry. The data were expressed as means and the differences among the groups were analyzed by ANOVA. The significance level was set at 0.05 with a 95% confidence level. Results: As much as 59.1% of the medicines presented pH lower than 5.5, being therefore potentially erosive to the dental tissues. Benflogin (1.75) and Predsim (7.35) presented the lowest and the highest pH means, respectively. As for TTA, the lowest mean was obtained for Cataflan (0.01%) and the highest for Infectrin (0.98%). Evaluation of o Brix revealed that Alersin presented the lowest mean (6.25%) and Cataflan (74.33%) presented the highest mean. There was no statistically significant difference among the medicine classes regarding the pH (p=0.950) or regarding the o Brix (p=0.477). On the other hand, there were statistically significant differences among the medicine classes for TTA (p=0.000); the greatest differences were found between the antibiotics and the anti-inflammatory drugs (p=0.002), the anthelmintics (p=0.003) and the corticoids (p=0.005). Conclusion:The pediatric medicines may be considered as potentially erosive to the dental tissues. In view of the statistically significant differences among the medicine classes regarding the TTA, it is recommended that the control and follow up be directed to the erosive potential of each class. The antibiotics are the class of medicine with the highest pH and acidity, and so its intake should be followed by an adequate oral hygiene. (74,33%). Não se verificou diferença estatisticamente significativa entre as variáveis pH e a classe de medicamento (P=0,950) e entre o °Brix e a classe de medicamento (P=0,477). Todavia, verificou-se diferenças estatisticamente significante em relação à acidez total e a classe de medicamento (P = 0,000), de modo que as diferenças maiores ficaram entre as médias dos antibióticos com: os antiinflamatórios (P=0,002), os antiparasitários (P=0,003) e os corticóides (P=0,005). Conclusão: Os medicamentos pediátricos analisados podem ser considerados potencialmente erosivos aos tecidos dentais. Em virtude das diferenças estatisticamente significativas entre as classes de medicamentos em relação à acidez total é necessário que o controle e o acompanhamento sejam direcionados ao potencial erosivo de cada classe, apontando os antibióticos como a classe com maiores médias de pH e acidez, devendo a sua administração ser controlada e acompanhada por uma adequada higiene oral. Efeito sobre a morfologia do esmalte dental e análise físico-química de medicamentos utilizados por pacientes pediátricos com paralisia cerebral ResumoIntrodução: Muitos medicamentos pediátricos líquidos são considerados fatores de risco para a estrutura dentária. Esse potencial pode aumentar quando ingeridos de forma crônica, como é o caso do tratamento de algumas condições, tal como a paralisia cerebral. Objetivo: Avaliar o efeito sobre a morfologia do esmalte dental e as propriedades físico-químicas de medicamentos de uso contínuo indicados a pacientes infantis com Paralisia Cerebral. Material e método: A amostra foi constituída por quatro medicamentos: Fenobarbital (M1), Carbamazepina (M2), Oxcarbazepina (M3) e Domperidona (M4), todos em suspensão oral. Analisaram-se o pH, o teor de Sólidos Solúveis Totais (SST) e a viscosidade dos medicamentos. Todos os testes foram realizados em duplicata. Para análise em microscopia eletrônica de varredura (MEV), 15 blocos de esmalte bovino foram distribuídos aleatoriamente e imersos em oxcarbamazepina (n=5), carbamazepina (n=5) e saliva artificial (n=5). Os ciclos de imersão foram feitos por seis dias, durante cinco minutos, duas vezes ao dia, com intervalos de 12 horas, quando ficavam mantidos em saliva artificial. O grupo controle permaneceu em saliva artificial. Os dados foram analisados descritivamente. Resultado: Em relação ao pH endógeno, os valores variaram de 2,82 (M3) a 9,60(M1). Para o SST, as médias de maior e menor valor foram, respectivamente, de 20,5% (M3) e 46% (M1). A viscosidade variou de 6,89 mm 2 /s (M1) a 58 mm 2 /s (M3). Em MEV, observaram-se alterações sugestivas de perda de estrutura no esmalte dental em oxcarbamazepina e carbamazepina, proveniente da ação dos medicamentos analisados. Conclusão: Concluiu-se que os medicamentos líquidos pediátricos analisados, indicados a pacientes com paralisia cerebral, apresentam potencial cariogênico e erosivo, destacando-se a Oxcarbazepina.Descritores: Paralisia cerebral; uso de medicamentos; erosão dentária; cárie dentária. This potential may increase when taken chronically as is the case of some conditions such as cerebral palsy. Objective: To evaluate the effect on the morphology of dental enamel and the physicochemical properties of continuous use of drugs given to pediatric patients with cerebral palsy. Material and method: The sample consisted of three drugs for epilepsy: Phenobarbital (D1), Carbamazepine (D2), Oxcarbazepine (D3); and one drug for gastroesophageal reflux: Domperidone (D4). The presence of damage associated with cerebral palsy (CP) often requires the long-term continuous use of prescription drugs. Among the adverse side effects of these drugs are decreased salivary flow and increased risk for caries. Objective: To determine whether those responsible for CP patients received orientation regarding the necessity of oral hygiene after the administration of drugs of continuous oral use and the caries experience in patients with CP. Method: Participating in this study were 205 children with medical diagnosis of CP, of both genders, aged 0-12 years (6.6 ± 2.9) attending a preventive program in dentistry in a reference institution of rehabilitation in São Paulo-SP. The data regarding gender, movement disorder and clinical types of CP were collected from medical records. In an interview format, the caregivers reported whether there was use of any continuous medicine. They were asked what form it took, whether the caregivers had received prior guidance on the importance of oral hygiene after the use of medicines, who performed the oral child's hygiene, and how often it was done. Their caries experience was recorded based on whether a given tooth was decayed (D, d) missing (M, m) or filled (F, f) (DMFT). The sample was divided into two groups according to whether they used continuous drugs (group 1: G1) or did not (group 2: G2). The Chi-square test and Student's t test were used, with the level of significance set up at 5%. Results: G1 (n = 110) and G2 (n = 95) were similar in terms of gender (p = 0.343) and age (p = 0.514). However, they differed significantly in relation to the clinical pattern, with G1 presenting significantly higher percentages of patients with tetraparesis (p < 0.001). Considering only the G1, it was observed that the subgroup that used medicine in oral solution form (solution, n = 65) differed significantly from that using tablets (tablets, n = 45) in relation to their previous orientation for hygiene oral (p = 0.013), in that the solution subgroup had been better oriented. With regard to oral hygiene, the solution subgroup had significantly higher percentages (p = 0.044) of children requiring supervision and completion of the oral hygiene when compared to the tablet group. The comparison of caries experience between G1 and G2 showed that the G1 had significantly higher values for their DMF index (p = 0.048), and fewer caries-free patients (p = 0.016) when compared to G2. Conclusion: Although those responsible for the patients receiving medication in the form of oral solution were better oriented, these patients presented higher values of caries experience.
