Introduction: To analyse the results of Cauda Equina Syndrome (CES) operated by Percutaneous Transforaminal Endoscopic Lumbar Discectomy (PTELD). Material and Methods: The study is a retrospective series of 15 patients operated by PTELD. Bladder dysfunction was classified as incomplete CES (CESI) and complete CES retention (CESR). Bladder / motor recovery rate and its timing, Oswestry Disability Index (ODI), Visual Analogue Score (VAS), patient satisfaction index, and sexual dysfunction were used to measure the outcome objectively. Additionally, in CESR patients, post-void residual (PVR) urine was measured by sonography. Complications and technical problems were noted. Results: There were ten patients of CESI and five patients of CESR. The average follow-up was 20.33(12.05) months. Bladder symptoms recovery was 100%, and motor recovery was 80%. VAS for back pain recovered to 0.53(0.52) from 8(2.39). VAS for leg pain recovered to 0.13(0.35) from 9.20(1.32). ODI improved to 6.07(2.85) from 77.52(13.20). The time to the recovery of bladder function was 1.47(1.55) days. All CESR patient’s abnormal PVR urine was normalised at five weeks post-operative. No complications were reported. However, five technical executional problems occurred. Conclusion: PTELD can be considered for CES treatment due to its substantial and quick recovery advantages. However, more evidence support is needed to make it a practice recommendation.

ObjectiveThe primary objective of this study was to analyze the characteristics and outcomes of cases admitted to hospital with cauda equina syndrome (CES) at the Institute of Orthopedics and Traumatology (IOT) from 2005 to 2015. Secondly, this article is a continuation of the epidemiological work of the same base published in 2013, and will be important for other comparative studies to a greater understanding of the disease and its epidemiology.MethodsThis was a retrospective study of the medical records of admissions due to CES at IOT in the period 2005–2015 with diagnosis of CES and neuropathic bladder. The following variables were analyzed: gender, age, etiology of the disease, topographic level of the injury, time interval between injury and diagnosis, presence of neurogenic bladder, time interval between diagnosis of the CES and surgery, and reversal of the deficit or of the neurogenic bladder.ResultsSince this is a rare disease, with a low global incidence, it was not possible, just with the current study to establish statistically significant correlations between the variables and outcomes of the disease. However, this study demonstrates the shortcomings of the Brazilian public health system, both with the initial management of these patients and the need for urgent surgical treatment.ConclusionThe study shows that despite well-defined basis for the conduct of CES, a higher number of sequelae caused by the pathology is observed in Brazil. The delay in diagnosis and, therefore, for definitive treatment, remains as the major cause for the high number of sequelae. Level of evidence: 4, case series.

Objetivo: Relatar um caso de hérnia de disco em paciente feminino jovem, que evoluiu com Síndrome da Cauda Equina (SCE) e necessidade de abordagem cirúrgica imediata. Detalhamento do caso: Paciente feminina, 15 anos, encaminhada ao hospital de referência da cidade de Franca-SP, com quadro de dor lombar modera há 3 semanas e piora nos últimos 15 dias, irradiada para membros inferiores – predominantemente à direita e evoluiu com disfunção urinaria e fecal. A Ressonância Magnética (RM) da coluna lombossacra evidenciou hérnia discal L5-S1. Paciente foi submetida imediatamente a laminectomia bilateral e artrodese com estabilização do quadro. A coleta de informações foi autorizada pela paciente e seu responsável legal através do termo de consentimento livre e esclarecido (TCLE) o qual foi subjugado e aceito pelo comitê de ética da fundação Santa Casa de Misericórdia de Franca com o CAAE 00474918.60000.5438. Considerações finais: A SCE é uma emergência neurocirúrgica cujo tratamento cirúrgico tem como objetivo o alívio da dor, a recuperação e preservação neurológica e o restabelecimento funcional.
