A peripheral nerve injury (PNI) can result in motor or sensory disorders. Low‐level laser therapy (LLLT) has demonstrated positive results as a treatment option for PNI. Wistar rats were divided into five groups: Control, Injury, Injury + LLLTn (nerve), Injury + LLLTm (muscle) and Injury + LLLTn + m (nerve and muscle irradiation). The groups were analyzed after one, two, three and four weeks. PNI was achieved by crushing the sciatic nerve. Laser treatment (780 nm, 3.2 J) was realized over the nerve and/or tibialis anterior muscle. In gait analyses, the groups irradiated over the nerve demonstrated an improvement after two weeks. In the analysis of mechanical sensitivity, the Injury + LLLTn demonstrated a reduction after one week in comparison with Injury group; the Injury + LLLTn + m and Injury + LLLTm demonstrated an increase after two weeks in comparison with Injury group; and the irradiated groups demonstrated a reduction in nociception after four weeks in comparison with Injury group. In the analysis of muscle atrophy, the Injury + LLLTn demonstrated more muscle mass after two weeks. LLLT improves functional aspects related to gait, mechanical sensitivity and muscle mass, with better results regarding motor aspects and muscle mass when administered over the injured nerve and better results regarding sensory aspects when administered over the muscle.

ResumoOs nervos periféricos são comumente acometidos por lesões. Dentre as causas mais comuns, estão: traumas, estiramento, pinçamento, compressão e esmagamento do tecido nervoso, que resulta em comprometimentos motores, sensoriais e limitações funcionais. Existem diversas formas de tratamento, dentre as abordagens terapêuticas não cirúrgicas, pode-se citar o laser de baixa potência, o qual apresenta uma série de indicações, desempenhando ação analgésica, anti-inflamatória e cicatrizante. Entretanto, há uma aparente carência na literatura brasileira com relação ao efeito do laser da faixa de luz visível (luz vermelha) como modalidade de tratamento para lesão nervosa periférica, bem como, a falta de revisões sistemáticas publicadas. Desta maneira, o objetivo deste estudo foi realizar uma revisão sistemática sobre o laser de baixa potência, no espectro de luz vermelha em lesão nervosa periférica. Para a realização deste estudo foram consultadas as bases de dados Scielo, Lilacs e Google Acadêmico, com as seguintes palavras chaves: Laser de baixa potência; Laser de baixa potên-cia e Lesão nervosa periférica; Laser de baixa potência 660 nm na lesão nervosa periférica e "Laser de 670 nm" "ciatalgia". Com isso, foram encontrados 20 estudos, mas, devido aos critérios de inclusão e exclusão, foram selecionados apenas cinco. Após análise dos mesmos, concluímos que o laser de baixa potência, no espectro de luz vermelha em lesão nervosa periférica em dois dos cinco artigos inclusos, por meio de literatura nacional demonstrou melhorar o processo de regeneração nervosa, porém os parâmetros utilizados são variados o que dificulta uma comparação entre os estudos.Palavras-chave: Terapia a laser de baixa intensidade; Traumatismos dos nervos periféricos; Regeneração Nervosa.• Artigo submetido para avaliação em 16/12/2014 e aceito para publicação em 26/03/2015 •

On‐Site Laser Photobiomodulation Treatment of Crushed Muscle Due to Prolonged Pressure in Rats

Background and Objectives: Crush injuries and prolonged pressure on muscles lead to bruises and sprains and, in most of the cases, cause distraction of the muscle and release of particles into the blood stream, causing renal and systemic complications in severe cases. Laser photobiomodulation treatment (i.e., laser phototherapy) is a method suggested to decrease the pressure damage in the first 24-48 hours after muscle injury, allowing a faster and more complete physical rehabilitation. We studied the efficacy of non-invasive laser photobiomodulation treatment as an on-site treatment for crush-injured gastrocnemius muscles, developing a moderate muscle crush injury model and aiming at decreasing damage extent while regaining physical competence faster. Study Design/Materials and Methods: Muscle crush injury was performed on 30 female Wistar rats using direct pressure for 10 minutes on the gastrocnemius muscle in both left and right hindlimbs. Immediately after the injury, only the left hindlimb were irradiated for 16 minutes (with 780 nm laser with a power of 250 mW, the energy at the target was 240 J, and the fluence was 1019 J/cm 2 ) for 1, 3, or 7 consecutive days, and sacrificed accordingly. During the follow-up period, 1, 3, or 7 days, both gastrocnemius muscles (of the treated and untreated hindlimbs) were evaluated for electrophysiology and functionality. Results: The laser photobiomodulation treatment showed a significant electrophysiological and functional recovery of the gastrocnemius muscle during the first 3 days after injury, in comparison with the untreated hindlimb. Conclusions: These preliminary results are promising, showing a significant effect of the laser photobiomodulation treatment during the first 3 days after the induction of the muscle crush injury, which is the most critical period in the clinical aspect. These findings suggest a therapeutic approach, which may help restore the muscle after crush injury.
