BackgroundDry needling (DN) and percutaneous electrical nerve stimulation (PENS) are widely used techniques in the treatment of myofascial pain.ObjectiveTo investigate the immediate and short-term effects of the combination of DN and PENS compared to DN alone on the upper trapezius muscle.MethodThis is a 72-hour follow-up single-blinded randomized controlled trial. Sixty-two volunteer patients with chronic myofascial neck pain with active Myofascial Trigger Points (MTrPs) in the upper trapezius muscle were recruited. Randomization was performed, and 31 patients received DN treatment (DN group) and 31 received DN and PENS (DN+PENS group). The primary outcomes were neck disability index (NDI) and visual analog scale for pain for both post-needling soreness (PNS) and neck pain intensity (NPI). Pressure pain threshold (PPT) and cervical range of motion (CROM) were the secondary outcomes. ResultsWe detected between-group differences in NPI and PNS in favor of the DN+PENS group immediately after treatment. No between-group differences in NDI were observed.ConclusionPENS application after dry needling treatment is more effective than dry needling alone for decreasing soreness in the short term and improving neck pain intensity immediately in patients with myofascial chronic neck pain.

BackgroundMultiple sclerosis (MS) is a complex autoimmune disease mediated by an immune response to central nervous system antigens. Modern immunomodulatory therapies, however, do not ameliorate many of the symptoms, such as pain and depression. Patients thus seek alternative treatments, such as acupuncture, although the benefits of such treatments have not been objectively evaluated. The present study was thus designed to evaluate the effect of the use of acupuncture in the alleviation of the symptoms of patients with MS.MethodsThirty-one patients with Relapsing-Remitting Multiple Sclerosis undergoing treatment with immunomodulators were randomly distributed into sex-stratified experimental and placebo groups in a patient- and evaluator-blind design; they received either true or sham electroacupuncture during regular visits to the doctor in the university hospital outpatient clinic. Standardized questionnaires were used to evaluate the effect of electroacupuncture on the quality of life of these patients. Initial and follow-up assessment included the evaluation of clinical status (Expanded Disability Status Scale), pain (Visual Analogue Scale) and quality of life (Functional Assessment of multiple Sclerosis) to ascertain the impact of electroacupuncture on the quality of life of these patients.ResultsElectroacupuncture improved various aspects of quality of life, including a reduction in pain and depression. The self-report scales were more sensitive to improvement than was the more objective clinical measure.ConclusionThis paper provides evidence that electroacupuncture can significantly improve the quality of life of such patients. The results suggest that the routine use of a self-report scale evaluating quality of life should be included in regular clinical evaluations in order to detect changes more rapidly.Trial RegistrationRBR-58yq52

Resumo:Contextualização: A cervicalgia é um quadro doloroso comum e que conta com diversas formas de tratamento, incluindo a acupuntura. Objetivo: Compilar, organizar e discutir sobre os resultados de pesquisas científicas sobre a acupuntura e outras técnicas da Medicina Tradicional Chinesa (MTC) no tratamento de cervicalgias. Métodos: Utilizou-se as bases de dados Medline, Lilacs, Scielo e PEDro para buscar artigos na língua inglesa, espanhola e português com as palavras-chave: acupuncture, cervicalgy, cervical pain, neck pain, auriculotherapy, ear therapy, auriculartherapy, YNSA, koryoacupuncture, quiroacupuncture e eletroacupuncture. A busca foi realizada no período de Março a Junho de 2016. Foram atribuídos os seguintes critérios de elegibilidade: indivíduos adultos, quadros de cervicalgia aguda ou crônica, uso de técnicas da MTC e avaliação da dor antes e após a intervenção. Estudos com cervicalgia de origem específica, técnicas não contempladas pela MTC e a não descrição da técnica utilizada foram rejeitados. Resultados: Foram encontrados 35 artigos, porém foram utilizados 13 destes artigos, que correspondiam aos critérios de inclusão. Observou-se maior utilização da acupuntura sistêmica (46% dos estudos selecionados) e dos acupontos VB21 (90,91%), VB20 (54,55%), IG4 (54,55%), ID3 (36,36%) e B10 (36,36%). A dor dos participantes foi principalmente avaliada pela escala EVA. Conclusão: Todos os artigos apresentaram resultados positivos quanto ao uso de acupuntura, eletroacupuntura, ventosaterapia e auriculoterapia no tratamento da cervicalgia, porém há a necessidade de mais ensaios clínicos que mostrem evidências científicas para consolidar os resultados observados e unificar a metodologia de avaliação da dor cervical, com ferramentas que contemplem os diversos aspectos da dor.Palavras-chave: Cervicalgia, Acupuntura, Eletroacupuntura, Dor.Abstract: Background: Neck pain is a common pain and it has many forms of treatment, including acupuncture. The following eligibility criteria were attributed: adult individuals, acute or chronic neck pain, use of TCM techniques and evaluation of pain before and after the intervention. Studies with cervicalgy with specific origin, techniques not contemplated by the TCM and the non description of the technique used were rejected. Results: : 35 articles were found, but 13 of these articles were used, which corresponded to the inclusion criteria.
