Rheumatoid arthritis (RA) is an autoimmune systemic complaint. Costus speciosus, an ornamental plant, which traditionally used in urinary diseases, rheumatism, jaundice and dropsy. The purpose of the study is to assess the protective effect of Costus on rheumatoid arthritis induced male rat in fertility. Thirty male adult Wistar rats (190-200 g) were sectioned into 6 groups. They were subdivided into 3 groups; group 1 was control received Distilled water, groups II and III received two various doses of Costus extract (200 and 400 mg/kg, respectively) for 60 days. Another 3 groups were subjected to RA induction via Freund's adjuvant. First group of RA induced rats was given Distilled water. Other two groups were given orally (200 and 400 mg/kg dosage of extract, respectively) from the 2 nd day of RA induction for 60 days. Sex organ relative weight, sperm concentration assay, testicular histopathology and immunohistochemistry of androgen receptors, TNF α and Bax protein were determined. Rheumatoid arthritis caused signi cant decrease in relative weight of sex organs which relatively improved by doses of Costus (200, 400 mg/kg). In addition, RA caused a signi cant reduction in sperm count which was improved via Costus (200, 400 mg/kg). RA induction caused testicular degeneration with marked depletion of spermatozoa, which markedly improved with costus treatment as shown in histopathological sections. Rheumatoid arthritis caused a reduction in %IHC of androgen receptors and increase of %IHC of both TNF α and Bax protein. Costus (200, 400 mg/kg) signi cantly improved %IHC of androgen receptors and signi cantly decreased %IHC of both TNF α and Bax protein, in RA induced groups. We can concluded that Costus speciosus had potentially useful role in improving fertility disorders caused by RA.

Resiliência é um termo originalmente empregado na Física que, posteriormente, passou a ser utilizado também pela Psicologia para descrever a capacidade do indivíduo de recuperar, no contexto de saúde mental, sua própria homeostase e evitar o desenvolvimento de um resultado psicopatológico após exposição aguda ou crônica ao estresse. Um indivíduo com alta resiliência tem, portanto, maior capacidade de manejar suas dores e problemas e, consequentemente, adotar uma postura positiva diante, por exemplo, do diagnóstico de doenças crônicas, como as doenças reumáticas autoimunes. OBJETIVO: Avaliar o grau de resiliência de pacientes com Lúpus Eritematoso Sistemico (LES) Artrite Reumatóide (AR) e Esclerose Sistêmica (ES) acompanhados no ambulatório especializado de um hospital universitário, buscando identificar os possíveis fatores associados à baixa resiliência e investigar a relação desta com depressão e ansiedade. MÉTODOS: Estudo observacional transversal e descritivo, utilizando-se de questionários de resiliência (Escala de Resiliência de Wagnild Young), ansiedade e depressão (Escala Hospitalar de Ansiedade e Depressão – HADS) em pacientes acompanhados no ambulatório especializado de reumatologia. RESULTADOS: Foram incluídos neste estudo 44 pacientes (23 com LES, 17 com AR, 4 com ES). Destes, 27,3% tinham grau de resiliência baixo e a prevalência de depressão foi 16,25 maior nestes pacientes do que aqueles com grau moderado e 13,75 vezes maior comparado aos de grau elevado. Também apresentaram prevalência 9,78 vezes maior de distúrbios de ansiedade do que pacientes com grau moderado, e 16,50 vezes maior do que pacientes com grau elevado de resiliência.CONCLUSÃO: Pacientes

The birth of reproductive rheumatology as a subject of interest in rheumatology has led to improvement of clinical care for patients living with autoimmune rheumatic diseases and paved the way towards setting a specialized pregnancy service within the standard rheumatology practice. In contrast to women, where there has been wealth of literature regarding pregnancy, lactation, and birth outcomes, there is not as much focusing on male sexual health and outcomes among inflammatory arthritis patients. Challenges such as decrease ability to conceive, impaired fertility, erectile dysfunction, and other sexual problems have been raised by male patients living with autoimmune rheumatic diseases. This broad scope gives the reproductive health concept in men another expansion with views to include sexual health problems screening among men attending the standard outpatient rheumatology clinics. This article adds to the paucity of real-life experience and aims at discussing the sexual health from the men perspective and provides a practical approach towards screening, and assessment of men living with autoimmune diseases in standard day to day practice.
