Neck circumference, a bedside clinical feature related to mortality of acute ischemic stroke

Abstract: Poor sleep quality, excessive daytime sleepiness and high risk of OSA are frequent in this sample with acute ischemic stroke. One-year mortality was related to older age and large NC. As obesity is uncommon in acute stroke patients, a large NC should be taken as a significant clinical sign related to mortality.

“…When we defined obesity as BMI above 25, obese patients had much higher value of NC, suggesting the relation of obesity with neck circumference (data not shown). NC, one clinical characteristics related to higher mortality of acute ischemic stroke, has been shown to be an independent predictor of coronary artery disease 29,30. NC was identified in our study as a significant clinical characteristic by multivariate regression analysis and was associated with the presence of hypertension in moderate to severe OSA patients.…”

“…NC was identified in our study as a significant clinical characteristic by multivariate regression analysis and was associated with the presence of hypertension in moderate to severe OSA patients. In recent studies, rostral fluid displacement in hypertensive patients has been linked to OSA by contributing to the narrowing of the upper airway,31,32 and NC could be increased by this fluid shift from the lower body 22,29,30,33,34. This result suggests that NC should not be overlooked in evaluating OSA patients.…”

“…[ 10 , 14 , 26 30 ] Several recent studies demonstrated that NC was a significant risk factor for type 2 diabetes, cardiovascular diseases, and ischemic stroke. [ 31 ] In addition, a cross-sectional association between NC and eGFR was found in a group of patients with or without CKD, suggesting a possible pathogenic role of upper-body subcutaneous fat in the development of renal dysfunction. [ 29 ] In the present study, we showed that NC was a significant predictor of incident CKD in a longitudinal analysis of subjects who did not have CKD at baseline, which was consistent with the results of a previous study.…”

“…In a follow-up study involving acute ischemic stroke patients who have a 1-year total mortality of 8.9%, the author discovered that aging and larger neck circumference were more frequent findings among the dead, but not obesity. Therefore, NC is a critical clinical warning factor for fatal outcomes in acute ischemic stroke [ 25 ]. Insulin resistance, related with NC, causes arterial stiffness, which in turn has an impact on CKD and even nondiabetic CKD.…”
