O objetivo deste estudo foi descrever as complicações da candidíase sistêmica em recém-nascidos admitidos na Unidade de Terapia Intensiva Neonatal (UTIN), durante um período de 10 anos. Um estudo observacional foi realizado com 60 recém-nascidos admitidos na UTIN 1994-2003. A freqüência de candidíase foi de 1,8% e houve um predomínio do sexo masculino recém-nascidos prematuros. Baixo peso ao nascer ocorreu em 63,3% dos recém-nascidos, com 50% deles com peso inferior a 1500 gramas, e dentre esses, 23,3% pesaram < 1000g. Os achados clínicos mais freqüentes foram os sintomas respiratórios, anormalidades na temperatura, letargia, hepatomegalia e esplenomegalia. As espécies de Candida identificadas foram C.albicans (83,3%), C. tropicalis (6,7%), C.parapsilosis (5%) e C.glabrata (1,7%). Ocorreram complicações em 85% dos casos: infecção do trato urinário (66,7%), pneumonia (56,7%), meningite (13,3%), endocardite (13,3%), trombose venosa profunda (10%), endoftalmite (6,7%) e abscesso renal (1,7%). A taxa de mortalidade foi de 33,3%. Os autores concluíram que a freqüência de complicações por candidíase sistêmica foi alta, com lesões detectadas em vários órgãos e sistemas, o que provavelmente contribuiu para a alta mortalidade observada na população estudada. Os resultados obtidos mostram a importância da investigação sistemática de complicações multissistémicas em todos os recém-nascidos que tenham cultura positiva para Candida sp.

The aim of the present study was to analyse the bacteriological factors during the process of dental alveolitis, relating it to a higher incidence of bacteraemia in adult rats subjected to neonatal malnutrition. We used forty male Wistar rats, suckled by mothers fed a diet during lactation containing 17 % protein in the nourished group (N) or 8 % protein in the undernourished group (UN). After weaning, the animals were given the Labina standard diet. After 90 d, these animals underwent upper right incisor extraction and induction of alveolitis. The oral microbiota was obtained using a swab and blood culture through venous blood. These procedures were performed before the extraction, 5 min after extraction, on the 21st day after alveolitis for groups N-21 and UN-21 and on the 28th day after alveolitis for groups N-28 and UN-28. Data were expressed as means and standard deviations for parametric data, and as medians and interquartile intervals for non-parametric data. Statistical significance was considered by assuming a critical level of 5 %. Before and after extraction, lower bacterial growth was observed per colony-forming unit (CFU) in the perialveolar region of the upper right incisors of undernourished animals, while the opposite was true after alveolitis, when a larger number of CFU was observed in these animals. The percentage of positive blood cultures obtained after alveolitis was greater in the undernourished animals. The present study thus demonstrated the influence of neonatal malnutrition in the perialveolar microbiota and in the development of bacteraemia after dental alveolitis.

Disseminated candidiasis is the most common nosocomial fungal infection, and Candida albicans has been reported to account for 50% to more than 70% of cases of invasive candidiasis. However, recent reports have also suggested the emergence of infections caused by non-albicans species. In addition, less-common pathogenic yeasts (Malassezia, Trichosporon, Rhodotorula, Debaryomyces and Pichia) have recently been reported, with increased frequency, as causes of nosocomial infections with high mortality. This article describes two cases of fungemia caused by Pichia anomala in newborns that occurred in an intensive care unit (ICU), in November 2004 at the Instituto da Criança (Pediatric Institute) of the Hospital das Clínicas of the School of Medicine, São Paulo University, Brazil. The principal factors related to virulence (proteinase and phospholipase) and the susceptibility of the isolated strains to antifungal agents were also evaluated, and the biotype of each strain was determined through the use of an epidemiological marker (killer biotype).
