The objective is to identify functional alterations in the lungs, the existence of smoking related diseases (SRDs), possible contraindications and drug interactions during the pharmacological treatment of the dependence. The profile of the smoker, the level of nicotine dependence and the motivation to stop smoking are also evaluated at this time.This evaluation (Chart 1) should include accurate clinical history, complete physical examination, and some complementary tests, depending on local diagnostic resources.Chest X-ray is an essential tool during the treatment. A good physician-patient relationship, together with professional sensitivity and observation skills, will indicate the most appropriate time. Objective:To identify the actions taken by the Commission of Tobacco Control (CTC) to control smoking in the hospital environment. Methods: Descriptive and exploratory retrospective documentary research conducted at a university hospital in southern Brazil, in 2014. The content of the minutes of CTC meetings was used to create a database, and the rounds reports were descriptively analyzed. We sought to identify the most relevant actions from 2005 to 2014. Results: The CTC implemented the Tobacco-Free Environment programme restricted cigarette smoking to designated areas and subsequently deactivated these areas. The only remaining outdoor smoking area in 2014 was deactivated. Conclusion: CTC actions have contributed to tobacco control in the hospital environment. This study will hopefully serve as a model to encourage other institutions to implement similar actions. Keywords: Tobacco control campaigns. Tobacco smoke pollution. Smoking areas. RESUMO Objetivo:Identifi car as ações desenvolvidas por uma Comissão de Controle do Tabagismo (CCT) para controlar o fumo do ambiente hospitalar. Métodos: Pesquisa documental descritivo-exploratória retrospectiva, realizada em hospital universitário do sul do Brasil, em 2014. Construiu-se um banco de dados com o conteúdo das atas de reuniões da CCT e relatório das rondas realizadas, o qual foi analisado de forma descritiva. Buscou-se identifi car as ações mais relevantes no período de 2005 a 2014. Resultados: A CCT implementou o Programa Ambiente Livre do Tabaco, restringiu o consumo do cigarro aos fumódromos e posteriormente desativou os mesmos. Permaneceu somente uma área externa de tolerância ao fumo até 2014, a qual foi desativada. Conclusões: As ações da CCT contribuíram para controlar o fumo do ambiente hospitalar. Espera-se que este estudo sirva de incentivo e modelo para outras instituições. Palavras-chave: Campanhas para o controle do tabagismo. Poluição por fumaça de tabaco. Áreas destinadas ao tabagismo. RESUMO -OBJETIVO. O conhecimento da prevalência do tabagismo é necessário para a realizaA partir da Primeira Grande Guerra Mundial, iniciou-se um aumento considerável de consumo de tabaco na forma de cigarro, sendo que o consumo por pessoa cresceu rapidamente durante a primeira metade deste século, à taxa de 5 a 15% ao ano. Na década de 1950 foi publicado o primeiro relato científico relacionando tabagismo com cân-cer de pulmão. Nos Estados Unidos, o consumo de cigarros atingiu o pico em 1963. Em 1964, a publicação do primeiro relato do "Surgeon General" sobre Tabagismo e Saúde, concluiu que o tabagismo está causalmente relacionado ao câncer de pulmão e laringe nos homens, causa a bronquite crônica e está associado ao risco aumentado de várias outras doenças, incluindo a doença das artérias coronarianas 1,2 . Após esse relato, foi lançada a "Campanha anti-tabagismo", com esforços da saúde pública e das políticas de saúde para educação dos fumantes sobre os riscos pertinentes de fumantes foi a de 31 a 40 anos (26,6%). Não houve diferença estatística entre a prevalência de fumantes entre homens e mulheres. Em ambos os sexos, para todas as faixas etárias, os indiví-duos com maior nível de instrução fumavam menos. Das pessoas que responderam o questioná-rio, 82,5% estavam preocupadas em serem fumantes passivas. Dos fumantes, 55% estão pensando em largar de fumar e 42% julgam precisar de algum tipo de ajuda.CONCLUSÃO. Programas educacionais e de cessação do tabagismo em nível institucional são necessários e bem aceitos na UNIFESP, devendo ser realizados para que as leis de restrição do fumo dentro da instituição sejam efetivamente cumpridas.UNITERMOS: Tabagismo. Funcionários da saúde, docentes, alunos e enfermeiros. ao consumo de cigarros, assim como expansão do conhecimento científico acerca das conseqüências do tabagismo para a saúde, inclusive relacionadas aos indivíduos expostos ao fumo passivo.As campanhas anti-tabágicas nos Estados Unidos têm alterado os contornos da epidemia, verificando-se uma redução da prevalência de fumantes de 41% em 1965 para 24% em 1996, 9% entre médicos e menos de 1% entre médicos-pneumologistas [3][4][5][6] . No Brasil a prevalência de fumantes é de 32,6%, chegando a 40% na Região Sul 7 , com cerca de 20% de médicos fumantes e 8% de pneumologistas fumantes 8 .Este fato levou o cartel do tabaco a expandir suas atividades para o terceiro mundo, onde o consumo cresce. No Brasil, o consumo de cigarros cresceu até 7% ao longo da década de 1970, mas reduziu este ritmo a partir de 1980, fazendo com que o consumo "per capita" venha caindo ao redor de 1% ao ano na população com 15 ou mais anos de idade 9 .
