Meningite e endocardite infecciosa causada por Rhodotorula mucilaginosa em paciente imunocompetente

Abstract: The authors report the case of an immunocompetent man who presented with acute impairment of the neurological system, hypertensive crisis and renal failure. The patient was eventually diagnosed with Rhodotorula mucilaginosa meningitis and infective endocarditis. To the best of our knowledge, this is the first description of simultaneous infection of the meninges and endothelium caused by Rhodotorula in a non-immunocompromised patient.

“…9,25 It appears that Rhodotorula meningitis should be considered a hospital infection, although there is one case of community acquired infection was reported. 19 In our case, it is very difficult to hypothesise the pathogenesis of meningitis. Our patient was a known case of abdominal Koch's with retroviral disease.…”

“…18 Immunocompetent individuals are also at the risk of infection by this fungus, but with low prevalence. 19 Meningitis caused by Rhodotorula species is still rare. Tuon and Costa [18] have taken a systematic review of 128 cases of Rhodotorula infections.…”

“…R. mucilaginosa is the most frequent species causing fungemia, which is responsible for up to 79% of infections, followed by R. glutinis (7.7%). These infections are often associated with the presence of CVCs or others implantable medical devices, and especially occur in immunocompromised individuals [6,8,12,23].…”

“…Although, endocarditis by Rhodotorula spp. has already been related to immunocompetent patients [8, 9] apparently without risk factor for opportunistic infection, invasive infections caused by this yeast are mainly associated with underlying immunosuppression. The most affected are patients with severe diseases such as leukemia, cancer or other solid tumors, lymphoproliferative disease, HIV, diabetes mellitus and submitted to various types of surgery [1013].…”

“…Although it has been previously considered as a nonpathogenic microorganism, this agent is currently recognized as potentially harmful to health, especially in immunocompromised patients, often being directly associated with catheters, prostheses and grafts, endocarditis, peritonitis, and meningitis. More recently, this microorganism has been implicated as the causal agent of onychomycosis (LOSS et al, 2011).…”
