ABSTRACT:Objective: To prepare a dictionary in Portuguese for using in Iris and to evaluate its completeness for coding causes of death. Methods: Iniatially, a dictionary with all illness and injuries was created based on the International Classification of Diseases -tenth revision (ICD-10) codes. This dictionary was based on two sources: the electronic file of ICD-10 volume 1 and the data from Thesaurus of the International Classification of Primary Care (ICPC-2). Then, a death certificate sample from the Program of Improvement of Mortality Information in São Paulo (PRO-AIM) was coded manually and by Iris version V4.0.34, and the causes of death were compared. Whenever Iris was not able to code the causes of death, adjustments were made in the dictionary. Results: Iris was able to code all causes of death in 94.4% death certificates, but only 50.6% were directly coded, without adjustments. Among death certificates that the software was unable to fully code, 89.2% had a diagnosis of external causes (chapter XX of ICD-10). This group of causes of death showed less agreement when comparing the coding by Iris to the manual one. Conclusion: The software performed well, but it needs adjustments and improvement in its dictionary. In the upcoming versions of the software, its developers are trying to solve the external causes of death problem.

OBJETIVO: Identificar e comparar a escala de prioridades na saúde de idosos mais jovens 60 a 69 anos de idade e longevos 80 anos ou mais, segundo causas de mortalidade no Rio Grande do Norte, no período de 2001 a 2011. MÉTODOS: Estudo ecológico, cujos dados foram obtidos a partir do Sistema de Informações sobre Mortalidade SIM e medidos através da Mortalidade Proporcional MP. RESULTADOS: As doenças cardiovasculares são a principal causa de morte para os dois grupos, com MP de mais de 30%. Já o segundo capítulo mais importante se distingue entre os dois grupos, correspondendo às neoplasias 22,9% para os mais jovens, que ocupa a quarta posição entre os longevos 10,1%. As causas mal definidas são a segunda mais prevalente para estes 17,3% e a quarta entre os mais jovens 9,18%. Há expressivas diferenças em relação às doenças respiratórias, do aparelho digestivo e causas externas. Sobre a primeira, os mais jovens 12,8% têm MP duas vezes maior que os longevos 6,2%, enquanto estes possuem MP cerca de duas e três vezes maior para as duas últimas, respectivamente. CONCLUSÃO: O estudo indica a heterogeneidade da população idosa, produzindo demandas distintas para as ações em saúde. As doenças cardiovasculares representam a principal causa de óbito para os dois grupos, porém as neoplasias, doenças do aparelho digestivo e causas externas obtiveram maior valor de MP entre os idosos mais jovens, ao passo que as mal definidas e do aparelho respiratório são mais importantes para os longevos.

Breast cancer incidence and mortality in women under 50 years of age in Brazil

Many countries have reported an increase in breast cancer incidence in young women. The current study's objective was to explore breast cancer distribution in women less than 50 years of age in Brazil. A descriptive study on breast cancer incidence (selected cities) and mortality (Brazil and selected cities) in 2002-2004 was carried out, and the results were compared with those from other countries. The study also analyzed the trend in hospital morbidity and incidence rates for breast cancer. Porto Alegre (Rio Grande do Sul State) showed the highest incidence rates (17.9 and 165.5/100,000 in the 15-39 and 40-49-year age strata, respectively). Regarding mortality, Belo Horizonte (Minas Gerais State) showed the highest rate in the 15-39-year group and Porto Alegre in the 40-49-year group (2.8 and 25.5/100,000). Hospital admissions and incidence rates for breast cancer suggest a change in epidemiological distribution. The results reveal an epidemiological pattern of breast cancer in young Brazilian women with regional distribution characteristics.
