ObjectiveBiogeographic regionalization represents abstractions of the organisation of life on Earth, and can provide a large‐scaled framework for health management and planning. We aimed at determining a biogeographic regionalization for human infectious diseases in Brazil, and at investigating non‐mutually exclusive hypotheses predicting the observed regions.MethodsBased on the spatial distributions of 12 infectious diseases with mandatory notification (SINAN database, 2007–2020, n = 15,839), we identified regions through a clustering procedure based on beta‐diversity turnover. The analysis was repeated 1000 times by randomly shuffling the rows (0.5° cells) in the original matrix. We evaluated the relative importance of variables using multinomial logistic regression models: contemporary climate (temperature and precipitation), human activity (population density and geographic accessibility), land cover (11 classes), and the full model (all variables). We refined the geographic boundaries of each cluster by polygonising their kernel densities to identify clusters' core zones.ResultsThe two‐cluster solution showed the best correspondence between disease ranges and clusters geographic limits. The largest cluster occurred with more density in the central and northeastern regions, while the smaller and complementary cluster occurred in the south and southeastern region. The best model for explaining the regionalization was the full model, supporting the ‘complex association hypothesis’. The heatmap showed a NE‐S directional display of the cluster's densities, and core zones showed geographic correspondence with tropical + arid (NE) versus temperate (S) climates.ConclusionOur findings indicate that there is a discernible latitudinal pattern in the turnover of disease in Brazil, and this phenomenon is associated with an intricate interplay between contemporary climate, population activity, and land cover. This generalised biogeographic pattern may offer the earliest insights into the geographic arrangement of diseases in the country. We suggested that the latitudinal pattern could be adopted as a nationwide framework for geographic vaccine allocation.

Giant condyloma acuminatum, a rare variant of anogenital condyloma, shows rapid growth associated with immunodeficiency. Wound care after resection and outcomes were reported. NGS, black, 55 years, HIV positive, with giant condyloma acuminatum affecting from the groin to the intergluteal groove, which was resected, remaining the wound opened for later skin graft. Topical care included polihexametilene biguanide/betaine solution, essencial fatty acids solution, hydrofiber/silver, and poliuretane film. The wound developed secondary infection, so hidrofiber was replaced by polyurethane foam/silver/ibuprofen. There was improvement in infection and pain, contraction of the edges and the presence of granulation tissue across the lesion. In those conditions the skin graft was performed after 41 days. Despite possible confusion bias, it can be inferred that the care adopted prepared the wound bed to receive the skin graft. Keywords: Buschke-Lowenstein tumor; wound healing; wound infection; pain; fatty acids, essencial; anti-infective agents, local; betain; silver; ibuprofen. RESUMOO condiloma acuminado gigante, variante rara do condiloma acuminado anogenital, apresenta crescimento rápido associado a estados de imunodeficiência. Relatamos os resultados com os cuidados com a ferida operatória. Trata-se de homem de etnia negra, 55 anos, portador do vírus da imunodeficiência humana com condiloma acuminado acometendo desde as regiões inguinais até o sulco interglúteo, que foi ressecado permanecendo a ferida aberta para posterior enxertia. Os cuidados com essa ferida incluíram solução de polihexametileno biguanida/betaína, solução de ácidos graxos essenciais, hidrofibra/ prata e película. Evoluiu com infecção secundária sendo a hidrofibra substituída por espuma de poliuretano/prata/ibuprofeno. Houve melhora da infecção e da dor, contração das bordas e presença de tecido de granulação em toda a lesão. Naquelas condições, o enxerto de pele foi realizado no 41º dia. A despeito dos possíveis vieses de confusão, pode-se inferir que esses cuidados prepararam o leito da ferida para receber o enxerto de pele.

Diante das informações apresentadas é evidente a importância de averiguações regional e local sobre os acidentes com aracnídeos peçonhentos a fim de gerar dados a respeito dos danos ocasionados por esses animais ao ser humano e a contribuição das ações antrópicas para esse cenário.…”

Os acidentes provocados por animais peçonhentos foram incluídos na lista de Doenças Tropicais Negligenciadas por atingir populações de baixa renda, social e politicamente marginalizadas, que vivem em áreas rurais e urbanas. O objetivo dessa pesquisa é realizar a caracterização epidemiológica dos acidentes provocados por escorpiões e aranhas na região de Picos, Piauí. Os dados foram obtidos no Sistema de Informação de Agravos de Notificação (SINAN), no período de janeiro/2013 a dezembro/2023. Um total de 2.467 acidentes com aracnídeos foram contabilizados, destes o número de acidentes causados por escorpiões foi 11 vezes maior que aqueles provocados por aranhas. O ano de 2018 e os meses caracterizados pelo período chuvoso acumularam os maiores números de casos. Quanto aos aspectos sociodemográficos dos acidentados, 55,2% eram pessoas do sexo feminino e 53,4% de zona rural. Em relação ao grau de escolaridade registrado, os mais acidentados possuem o Ensino Fundamental Anos Iniciais incompleto. Referente à raça, 88% das vítimas se declararam parda. A maioria dos acidentes foram classificados como leves e o local do corpo mais afetado foi o pé. Quanto aos acidentes causados por aranhas, se destacaram os provocados pelo gênero Phoneutria. O número representativo dos acidentes ocasionados por aracnídeos peçonhentos na cidade de Picos, Piauí, observados na presente pesquisa, revelam um problema de saúde pública, especialmente durante os meses mais quentes e chuvosos. É urgente implementar medidas educativas para prevenir esses casos.
