Resumo Este trabalho teve como objetivo descrever um relato de experiência a partir de um Programa de Intervenção com mulheres em situação de infertilidade. Tal Programa faz parte do Projeto de Extensão da UEL/PR, na área da Psicologia, com fundamento na Análise do Comportamento. Foram realizados dois grupos, compostos por sete mulheres cada, de Londrina e região, entre 31 e 42 anos, em situação de infertilidade conjugal. Realizou-se 10 sessões, de aproximadamente duas horas cada, semanalmente. Após dois meses do término, realizou-se sessões de follow-up. Os procedimentos utilizados foram técnicas de dinâmica de grupo, técnicas da Análise do Comportamento, como modelagem e modelação, Terapia de Aceitação e Compromisso (ACT) e Psicoterapia Analítica funcional (FAP). Realizou-se avaliações qualitativas por meio de dados obtidos com os relatos das participantes. A partir da fala das participantes, puderam ser percebidas mudanças em seus comportamentos, tanto dentro das sessões quanto em seu ambiente natural, de acordo com seus relatos verbais. A intervenção grupal tem o potencial de promover a aprendizagem de um repertório de enfrentamento das dificuldades relacionadas à infertilidade, o fortalecimento de aspectos positivos no relacionamento conjugal e social, além da aceitação emocional e seu manejo. Durante as sessões de follow-up, quatro participantes relataram estar dando início ao processo de adoção e, cinco participantes conseguiram atingir a gravidez. Sugere-se a replicação deste Programa com este tipo de população, bem como estudos com controle de variáveis.

Background. In previous studies, people's knowledge of reproductive health and infertile women's psychological states was surveyed in several countries. However, there has been limited information concerning the psychological states of infertile women seeking treatment and the outcomes of in vitro fertilization (IVF) in China. Methods. Infertile women were asked to complete short questionnaires on the day that their oocytes were retrieved; these questionnaires covered the durations of their infertility, levels of education, sources of pressure, and psychological states. Data concerning IVF outcomes were provided by embryologists and clinicians. The correlations between the duration of infertility and educational level, psychological state and education level, and psychological state and outcome of IVF were analyzed in the cohort study. Results. The duration of infertility in more than half of the females was longer than 5 years. Compared with less-educated women, women with higher levels of education sought treatment earlier and their rates of depressive symptoms were lower. There is an association between negative emotions and outcome of IVF. Conclusions. The survey of the situations of infertile women seeking IVF treatment in China indicates the importance of popularizing knowledge concerning reproductive health. Improving medical conditions, reducing the costs of treatment, and developing social culture will aid in relieving the stress of infertile women and improving assisted reproductive treatment.

Бесплодие приводит к эмоциональному стрессу, что может стать фактором депрессии. В свою очередь стресс также признан одной из возмож-ных причин патоспермии и бесплодного брака. Целью пилотного исследования стала оценка влияние селективных ингибиторов обратного захвата серотонина (СИОЗС) на сперматогенез и фертильный потенциал в лечении пациентов с де-прессией. Материалы и методы. Исследование включило 19 фертильных муж-чин с депрессией средней степени выраженности. Данных за урологическую па-тологию выявлено не было. Пациентам был назначен флуоксетин в дозе 20 мг в сутки в течение 6 недель. Спермиологическое исследование (ВОЗ 2010), пока-затель фрагментации ДНК сперматозоидов оценены до, после окончания терапии и через 3 месяца после отмены препарата. Результаты. Все наблюдаемые имели нормальные показатели спермограммы и ДНК-фрагментации до лечения. Через 6 недель лечения мы зафиксировали снижение всех параметров спермограммы и увеличение индекса фрагментации ДНК сперматозоидов. Через 3 месяца после окончания терапии средний объем эякулята, концентрация, подвижность и про-цент нормальных форм сперматозоидов увеличились и практически вернулись к исходным цифрам. Средний показатель фрагментации ДНК снизился и имел значение меньшее, чем до назначения СИОЗС, что положительно коррелировало с динамикой симптомов депрессии. Выводы. Использование СИОЗС может не-гативно влиять на сперматогенез и фертильность мужчины в течение лечения. Наиболее выраженные изменения зарегистрированы в отношении морфологии и целостности структуры ДНК половых клеток. Через 3 месяца после отмены препарата отмечается возврат показателей к исходным значениям. Требуются дальнейшие исследования для оценки влияния СИОЗС в лечении бесплодных мужчин с депрессией. Introduction. Iinfertility leads to emotional stress, which is a precipitating factor of depression. Stress has also been recognized as one of possible reasons of pathospermia and infertility. The aim of pilot study: to evaluate SSRIs effects on spermatogenesis and fertility in the treatment of depression. Material and Methods. Study included 19 fertile men with depression of mild severity. None of them had any urological disorders. They were treated by fluoxetine 20 mg per day during 6 weeks. Semen analysis (WHO 5), level of DNA fragmentation were performed before, after the therapy and 3 months of drug discontinuation. Results. All the men had normal semen parameters and level of DNA fragmentation before the treatment.
