Interventional procedures, cine acquisitions and operation of fluoroscopic equipment in high-dose fluoroscopic modes, involve long fluoroscopic times which can lead to high staff doses. Also, Coronary angiography (CA) procedures require the cardiologist and assisting personnel to remain close to the patient, which is the main source of scattered radiation. Thus, radiation exposure is a significant concern for radiation workers and it is important to measure the radiation doses received by personnel and evaluate the parameters concerning total radiation burden. In this research, we investigated radiation doses to 10 cardiologists performing 120 CA procedures. Using thermo luminescent dosimeters doses to the wrists, thyroid and eyes per procedure were measured. Based on the measured dose values, maximum doses to the Left wrist, Right wrist, thyroid and eyes of cardiologist were measured 241.45 µSv, 203.17 µSv, 78.21 µSv and 44.58 µSv, respectively. The results of this study indicate that distance from the source, use of protective equipment’s, procedure complexity, equipment performance, and cardiologist experience are the principal exposure-determining variables. It can be conclude that if adequate radiation protection approaches have been implemented, occupational dose levels to cardiologists would be within the regulated acceptable dose limits.

Objetivo: relatar um caso de alopecia temporária após tratamento endovascular com exposição por fluoroscopia devido a uma malformação arteriovenosa na face. Detalhamento do caso: sexo masculino, 34 anos, com queixa de lesão na asa nasal, lábio superior e lateral da face (direita). O paciente trouxe exame de angioressonância apresentando uma malformação arteriovenosa em face com nutrição pela artéria facial e drenagem pela veia mandibular. Como tratamento foi optado uma arteriografia diagnóstica para melhor avaliação de vascularização da malformação arteriovenosa seguida de embolização com onyx® (mistura de etileno vinil álcool copolímero) que fornece o contraste necessário para a visibilização da mistura sob fluoroscopia. O procedimento foi realizado 14 dias após a 1ª consulta, sem intercorrências indicando sucesso terapêutico. No retorno, terceira semana após o procedimento, apresentou alopecia setorial em região occipitoparietal direita. Não havia manchas em região, bem como outros sintomas associados. Foi realizado como tratamento o uso de Minoxidil tópico e Cilostazol via oral. Após o tratamento houve retorno do crescimento espontâneo em cerca de 2 meses. Considerações finais: a embolização com onyx® mostrou-se uma valiosa opção terapêutica com uma maior conservação das estruturas nobres em malformações arteriovenosas, com baixa taxa de complicações no médio e longo prazo.

Interventional radiology (IR) uses minimally invasive image-guided procedures that may require a long duration and deliver high radiation doses to the patient. The most current studies on management and dose reduction in IR have shown to be of fundamental importance in radiation protection practices. The objective of this study was to survey what has been published in the national and international literature on the doses received by patients undergoing IR procedures. It consisted of an integrative review study carried out in Scielo, Pubmed, and INIS databases in the time frame of 2016-2021, using as descriptors: interventional radiology, radiation injuries, radiological protection, dosimetry; and their equivalents in English: interventional radiology, radiation injuries, radiological protection, and dosimetry. The initial sample consisted of 68 full articles related to the theme, published during the study period. Of these, 11 articles were selected and made up the final sample. The results indicate that the use of software in dose management and monitoring has proven to be an important tool in radiological protection. The implementation of dose reduction strategies and increased awareness about the use and effects of radiation result in significantly lower doses to both professionals and patients. No national study on dose management has been identified, highlighting the need to promote the subject in the country. The main recommendations identified were the introduction of technologies and the training of the workers involved.
