BackgroundMinimally invasive cardiovascular procedures have been progressively used in heart surgery. ObjectiveTo describe the techniques and immediate results of minimally invasive procedures in 5 years. MethodsProspective and descriptive study in which 102 patients were submitted to minimally invasive procedures in direct and video-assisted forms. Clinical and surgical variables were evaluated as well as the in hospital follow-up of the patients. ResultsFourteen patients were operated through the direct form and 88 through the video-assisted form. Between minimally invasive procedures in direct form, 13 had aortic valve disease. Between minimally invasive procedures in video-assisted forms, 43 had mitral valve disease, 41 atrial septal defect and four tumors. In relation to mitral valve disease, we replaced 26 and reconstructed 17 valves. Aortic clamp, extracorporeal and procedure times were, respectively, 91,6 ± 21,8, 112,7 ± 27,9 e 247,1 ± 20,3 minutes in minimally invasive procedures in direct form. Between minimally invasive procedures in video-assisted forms, 71,6 ± 29, 99,7 ± 32,6 e 226,1 ± 42,7 minutes. Considering intensive care and hospitalization times, these were 41,1 ± 14,7 hours and 4,6 ± 2 days in minimally invasive procedures in direct and 36,8 ± 16,3 hours and 4,3 ± 1,9 days in minimally invasive procedures in video-assisted forms procedures. ConclusionMinimally invasive procedures were used in two forms - direct and video-assisted - with safety in the surgical treatment of video-assisted, atrial septal defect and tumors of the heart. These procedures seem to result in longer surgical variables. However, hospital recuperation was faster, independent of the access or pathology.

Diante do crescimento tecnológico e avanços em cirurgia cardíaca, há alguns anos vem sendo introduzido no Brasil a técnica de cirurgia minimamente invasiva com o objetivo de aumentar a qualidade assistencial do cliente cardiopata, a fim de reduzir riscos, minimizar o trauma, o tempo de internação e oferecer uma melhor recuperação. Trata-se de uma revisão integrativa. Objetivou-se: identificar as evidências da literatura sobre a inserção da técnica minimamente invasiva em cirurgia cardíaca e suas vantagens. Obteve-se uma amostra de 11 artigos que atenderam aos critérios de inclusão. Evidenciou-se que a inserção da técnica minimamente invasiva é eficaz e segura para pacientes com doenças cardiovasculares, especialmente no que tange a redução do desconforto pós-operatório, melhorando os resultados estéticos, acelerando a recuperação pós-operatória, aliados í s baixas complicações.

Minimally invasive procedures are the standard approach in many centres but are still under debate in regards of inferiority compared to conventional mitral valve surgery through a median sternotomy. The aim of this review was to summarize the current literature comparing minimally invasive mitral valve surgery (MIVS) and conventional mitral valve surgery. In this review of the current literature, we summarize our findings from a recent meta-analysis and add information from papers that were published afterwards. There were no differences between patients treated minimally invasive or through a conventional sternotomy approach in regards of perioperative stroke rate and mortality. Procedural time, cardio-pulmonary-bypass time and cross-clamp time were longer in the MIVS group. In contrast, length of intensive care unit (ICU) stay and length of in hospital stay were significantly reduced in this group. Need for blood transfusion was lower in the MIVS group. Other outcomes like i.e., the rate of rethoracotomies or renal failure didn't differ between the groups. Repair rates and long-term freedom from recurrence of mitral regurgitation and reoperation are similar. Newer publications underline these findings. The current literature shows that MIVS and conventional mitral valve surgery show a similar perioperative outcome. Minimally invasive mitral valve surgery is favourable with regards to ICU stay, in hospital stay as well as need for blood transfusion.
