BackgroundAbnormal ankle-brachial index (ABI) has been found to be a strong predictor of mortality in some hemodialysis populations in studies with relatively short periods of follow-up, lower than 2 years.ObjectiveThis study aimed to assess the predictive value of abnormal ABI as a risk factor for death among patients on maintenance hemodialysis after a 5-year follow-up.MethodsA total of 478 patients on hemodialysis for at least 12 months were included in the study. ABI measurement was performed using a mercury column sphygmomanometer and portable Doppler. Patients were divided into 3 groups according to ABI (low: <0.9; normal: 0.9 to 1.3; and high: >1.3) and followed for a 60-month period.ResultsThe prevalence rates of low, normal and high ABI were 26.8%, 64.6% and 8.6%, respectively. The 5-year survival rate was lower in the groups with low ABI (44.1%, P<0.0001) and high ABI (60.8%, P= 0.025) than in the group with normal ABI (71.7%). Cox regression was used to evaluate the association between ABI and mortality, adjusting for potential confounders. Using normal ABI as reference, a low, but not a high ABI was found to be an independent risk factor for all-cause mortality (HR2.57; 95% CI, 1.84-3.57 and HR 1.62; 95% CI, 0.93-2.83, respectively).Conclusionslong-term survival rates of patients with either low or high ABI were lower than the one from those with normal ABI. However, after adjustment for potential confounders, only low ABI persisted as an independent risk factor for all-cause mortality among hemodialysis patients.

Left ventricular hypertrophy (LVH) is the most common target organs damage in the hypertension patients. Abnormal low (≤0.9) or high (≥1.40) ankle brachial index (ABI) are associated with an increased risk of cardiovascular events. However, the relationships between a high ABI in the normal range (0.9–1.4) and LVH in Han Chinese hypertension are not entirely elucidated. This study included 3953 hypertension patients aged 40–75 years among Han Chinese. Hypertension was defined as systolic blood pressure≥140 mm Hg, diastolic blood pressure≥90 mm Hg, or history of antihypertensive drug use. Left ventricular mass (LVM) was measured by transthoracic echocardiography. LVH was diagnosed by using the criteria of LVM ≥49.2 g/m2.7 for men and 46.7 g/m2.7 for women. Our study suggested that the ABI was higher in patients with LVH than in those without (1.13±0.11, 1.11±0.11, p < 0.001). The prevalence of LVH in patients with the lowest (0.9 < ABI≤1.03), second (1.04≤ABI≤1.11), the third (1.12≤ABI≤1.20), and the highest quartile (1.21≤ABI < 1.40) of ABI was 37.2%, 38.2%, 45.5%, 45.7%, respectively. Logistic regression analysis suggested that the highest and third quartile of ABI were significantly associated with increased LVH risk (multivariate‐adjusted OR of highest group:1.83; third group:1.61). The association of ABI at second quartile with LVH was nonsignificant. Similar results were observed in less than 60 years and without coronary heart disease or diabetes group. Our observations in Chinese patients with hypertension indicated high ABI may be an important risk factor for LVH in hypertension patients among Han Chinese, even in the normal range.

Introducción: la enfermedad arterial periférica es una afección frecuente en adultos mayores. Se acrecienta con la presencia de comorbilidades. Puede ser detectada por la medición de la presión arterial en los cuatro miembros y el cálculo del índice tobillo-brazo. Objetivos: determinar la frecuencia de enfermedad arterial periférica detectada por el índice tobillo-brazo, los síntomas de claudicación intermitente de miembros y la presencia de comorbilidades. Metodología: estudio observacional descriptivo prospectivo de corte transversal realizado en 62 pacientes mayores de 40 años internados en el Servicio de Clínica Médica del Hospital Nacional (Itauguá, Paraguay) en el 2016. Se determinó la presión arterial en los cuatro miembros con un tensiómetro digital y se registró la presencia de comorbilidades. El índice tobillo-brazo derecho se calculó por el cociente de la presión arterial sistólica del tobillo derecho dividido la presión arterial sistólica más elevada de cualquier brazo y de forma similar para el índice tobillo-brazo izquierdo. Se consideró índice tobillo-brazo compatible con enfermedad arterial periférica todo valor ? 0,9. Resultados: en la muestra predominó el sexo masculino (52%) y la edad media fue 54 ± 9 años. Las comorbilidades más frecuentes fueron hipertensión arterial (48%), tabaquismo (32%) y diabetes mellitus (26%). Se detectó enfermedad arterial periférica en 9,6%. La mayoría de los afectados se encontraban en el grado I de la clasificación de Fontaine (asintomáticos). Discusión: la frecuencia de enfermedad arterial periférica fue 9,6%. Sólo un paciente era sintomático. Las comorbilidades más frecuentes fueron la hipertensión arterial y el tabaquismo. La determinación de la presión arterial en los cuatro miembros permite detectar una enfermedad asintomática, potencialmente grave.
