Anatomical Variations in the Sinoatrial Nodal Artery: A Meta-Analysis and Clinical Considerations

Background and ObjectiveThe sinoatrial nodal artery (SANa) is a highly variable vessel which supplies blood to the sinoatrial node (SAN). Due to its variability and susceptibility to iatrogenic injury, our study aimed to assess the anatomy of the SANa and determine the prevalence of its anatomical variations.Study DesignAn extensive search of major electronic databases was performed to identify all articles reporting anatomical data on the SANa. No lower date limit or language restrictions were applied. Anatomical data regarding the artery were extracted and pooled into a meta-analysis.ResultsSixty-six studies (n = 21455 hearts) were included in the meta-analysis. The SANa usually arose as a single vessel with a pooled prevalence of 95.5% (95%CI:93.6–96.9). Duplication and triplication of the artery were also observed with pooled prevalence of 4.3% (95%CI:2.8–6.0) and 0.3% (95%CI:0–0.7), respectively. The most common origin of the SANa was from the right coronary artery (RCA), found in 68.0% (95%CI:55.6–68.9) of cases, followed by origin from the left circumflex artery, and origin from the left coronary artery with pooled prevalence of 22.1% (95%CI:15.0–26.2) and 2.7 (95%CI:0.7–5.2), respectively. A retrocaval course of the SANa was the most common course of the artery with a pooled prevalence of 47.1% (95%CI:36.0–55.5). The pooled prevalence of an S-shaped SANa was 7.6% (95%CI:2.9–14.1).ConclusionsThe SANa is most commonly reported as a single vessel, originating from the RCA, and taking a retrocaval course to reach the SAN. Knowledge of high risk anatomical variants of the SANa, such as an S-shaped artery, must be taken into account by surgeons to prevent iatrogenic injuries. Specifically, interventional or cardiosurgical procedures, such as the Cox maze procedure for atrial fibrillation, open heart surgeries through the right atrium or intraoperative cross-clamping or dissection procedures during mitral valve surgery using the septal approach can all potentiate the risk for injury in the setting of high-risk morphological variants of the SANa.

Introduction: Previous studies confirm that there is no anthropometric index of normal Iranian hearts, including diameter of cardiac valve, thickness of myocardial septum and coronary artery variation. The purpose of this study was to assess the anatomical indexes of the human heart and coronary artery in the Iranian population. Methods: The study was performed with 207 adult human hearts in both sexes (182 male and 25 female), fixed in 10% formaldehyde. Myocardial thickness and diameter of heart valves were measured using a Vernier caliper. Assessments of the coronary artery were carried out after removal of the pericardium. Results: Analysis of 207 cadaver coronary arteries showed left coronary artery (LCA) dominance type was present in 6.3% of corpus, and balance was present in 4.3% of corpus, while the largest number (89.4%) had right coronary artery (RCA) dominance. The mean values of the morphometric data are as follows: right atrial wall thickness (2.08 +/- 0.04 mm), left atrial wall thickness (2.08 +/- 0.04 mm), interatrial wall thickness (4.92 +/- 0.08 mm), right ventricular wall thickness (3.35 +/- 0.05 mm), left ventricular wall thickness (8.36 +/- 0.13 mm), interventricular wall thickness (12.01 +/- 0.2 mm), diameter of the aorta (23.6 +/- 0.4 mm), diameter of the pulmonary artery (24.94 +/- 0.4 mm), large diameter of the mitral valve (34.16 +/- 0.27 mm), and large diameter of the tricuspid valve (38.8 +/- 0.24 mm). Conclusion: The anatomical knowledge of cardiac indicators and coronary artery data will be helpful and clinically relevant, especially for cardiac surgeons for coronary artery bypass grafting and coronary arteriography.

The objective of the present meta‐analysis was to evaluate recent and applicable data regarding the location and variation of the atrioventricular nodal artery (AVNA) in relation to adjacent structures. In order to minimize postoperative risks and maintain physiological anastomosis for proper cardiac function, understanding such possible variations of vascularization of the AV node is of immense importance prior to cardiothoracic surgery as well as ablations. In order to perform this meta‐analysis, a systematic search was conducted in which all articles regarding, or at least mentioning, the anatomy of the AVNA was searched. In general, the results were based on 3919 patients. AVNA was found to originate only from the RCA in 82.41% (95% CI: 79.46%–85.18%). The pooled prevalence of AVNA originating only from LCA was found to be 15.25% (95% CI: 12.71%–17.97%). The mean length of AVNA was found to be 22.64 mm (SE = 1.60). The mean maximal diameter of AVNA at its origin was found to be 1.40 mm (SE = 0.14). In conclusion, we believe that this is the most accurate and up‐to‐date study regarding the highly variable anatomy of the AVNA. The AVNA was found to originate most commonly from the RCA (82.41%). Furthermore, the AVNA was found to most commonly have no (52.46%) or only one branch (33.74%). It is hoped that the results of the present meta‐analysis will be helpful for physicians performing cardiothoracic or ablation procedures.
