A elevação da pressão arterial sistólica durante o teste ergométrico após transplante cardíaco: correlação com o quadro clínico e a função ventricular avaliada pela ecocardiografia sob estresse com dobutamina

Abstract: Unlike other populations, no correlation was found between delta SBP during exercise testing and clinical condition or left ventricular function in heart transplant patients. Pathophysiological factors associated with delta SBP reduction during exercise testing remain unknown.

“…Autonomic nervous system improvement may explain it [24,43,44]. The average increase of SBP peak should be 50% of the resting value and an insufficient increase has been associated with left ventricular systolic dysfunction [45]. Nevertheless, the relationship between SBP response and V O 2 peak is unclear.…”

“…By use of dobutamine, Bellotti et al 28 have reported normal contractility in transplanted patients with no rejection; however, in the presence of rejection, contractility was reduced 29 , suggesting that dobutamine echocardiography could aid in correlating with episodes of rejection.…”

“…Left ventricular function, evaluated through the Doppler echocardiogram, consists of the filling and emptying of the left ventricle, which determines the LVEF (%) and consequently translates the ability of the left ventricle to eject blood into the aorta during systole. 38 The LVEF (%) involves both the final systolic volume (FSV) and the final diastolic volume (FDV), with values ≥55% (Teicholz method) being considered normal for healthy adults. 38 In cardiac transplant patients there is a reduction in systolic volume at rest and during exercise, when compared to healthy individuals.…”
