Importância da proteína C-reativa no diagnóstico e no prognóstico intra-hospitalar em pacientes com dor torácica na sala de emergência

Abstract: In patients with chest pain presenting at the emergency room, s-CRP was not a good marker of AMI, although this diagnosis is virtually excluded by a normal value; in addition, values one-third above the upper limit of normal (>1 mg/L for hs-CRP or >0.33 mg/dL for t-CRP) were predictive of in-hospital adverse cardiac events.

“…The exclusion criteria were as follows: BMI≥30 kg/m 2 , alcohol or smoking habit, diabetes, hypertension, use of illicit drugs or medications that could affect the responses of the variables, and for women, use of contraceptives or hormone replacement therapy. In addition, individuals with a history of inflammation and hsCRP serum values suggestive of acute inflammation were excluded, i.e., above the normal upper limit: 3.0 mg/L (16).…”

“…All volunteers were submitted to clinical and physical examinations, and a standard electrocardiogram (ECG) at rest and during maximum cardiopulmonary exercise test. Exclusion criteria were: ECG with alterations; abnormalities in the cardiovascular, respiratory, neurological, and osteomyoarticular systems; drinkers; smokers; obese; diabetic or hypertensive patients; users of illicit drugs or medications that could interfere with the responses of the studied variables, as well as women using contraceptive medication or hormone replacement therapy; subjects who presented high values of serum hsCRP (above the upper limit of normality of 3.0 mg/L [11] and suggestive of acute inflammation). The menstrual cycle phase was controlled for women of reproductive age, and the experiments were performed in the follicular phase, ranging from the end of menstruation to the midpoint of the cycle.…”

“…According to the findings in the Potsch et al ( 22 ) study, CRP levels were higher in patients with a primary diagnosis of AMI compared to those who did not have this diagnosis, so CRP values above the level at the time of admission are a useful tool in the identification of patients with more severe chest pain.…”

“…Regarding the analytical variables, CRP, defined as “a marker of acute inflammatory phase response synthesized in the liver” ( 22 ), has been associated with the assessment of cardiac risk in intermediate-risk patients ( 23 ).…”
