Sobrevida e fatores prognósticos na insuficiência cardíaca sistólica com início recente dos sintomas

Abstract: Evidence shows that mortality in the initial phase is not elevated, and that etiology, advanced functional class, arterial hypotension, tachycardia, presence of the third cardiac sound, and elevated serum creatinine lead to a worse prognosis.

“…Most of the publications on the importance of HF as the cause of death in Brazil are derived from cohort studies 9,10 or related to hospital mortality, based on the data available at the Brazilian Public Health System (SUS) regarding hospitalizations in public hospitals or those hired by the system 11,12 . In Brazil, HF was the third cause of hospitalization due to clinical causes through SUS/MH(Ministry of Health) from 1992 to 2002, with percentages of hospitalizations in relation to the total number of hospitalizations that varied from 3%-4%, with lethality rates of 5%-7% at hospital admissions 2 .…”

“…Another study yielded similar results. Rassi et al followed up a cohort of 204 patients with heart failure during 46 months and identified some risk factors for cardiovascular death, including NYHA functional classes III and IV (2.7 times higher), an increase of 10 bpm in heart rate (1.6 times higher), increased serum creatinine (60% higher), and Chagas disease (4.1 times higher) 5 .…”

“…In Mexico, the reported seroprevalence rates have ranged from 19.4% in Yucatán to 82.5% in Chiapas [14][15] ; in Brazil, the prevalence has been reported to range from 20% in São Paulo to 48% in Bahia [5][6][7][8][9] . All authors agree that seroprevalence is higher in HF patients; however, the point of interest is to ascertain whether the HF syndrome in seropositive patients is a consequence of CChC.…”

“…This poor prognosis has been associated with the presence of ventricular tachycardia, low ejection fraction, bradycardia, decreased systolic pressure, decreased VO 2 max, activation of the renin-angiotensin system, and elevated levels of cytokines [7][8] .…”
