Submitral left ventricular aneurysm is a rare cardiac pathology. Etiology remains still mysterious, but it is believed to be due to a congenital defect of the fibrous layer of the atrioventricular junction. Surgical closure of aneurysm rim is the therapy of choice, despite the diversity of surgical approaches and techniques. We present a young patient operated with a transmitral approach in our clinic. Posterior mitral leaflet (PML) detachment and then extension valvuloplasty by using autologous pericardium was performed in order to save the native mitral valve. We believe that respecting the integrity of native valve is of crucial importance in this young population of patients.

Submitral aneurysm is seldom reported worldwide. It is thought to be caused by weakness in the cardiac musculature due to abnormal embryogenesis. Mostly these aneurysms were reported from Africa in black African population. Apart from congenital cause, many other causes have been put forth as its etiology. Among these endocarditis is a known infective etiology. Submitral aneurysm secondary to left sided infective endocarditis is rare. The definitive diagnostic modality and treatment of submitral aneurysm remains echocardiography and surgical correction of aneurysm respectively. The submitral aneurysms as well as infective endocarditis both are known independent risk factors for thromboembolic events and subsequent stroke increasing both morbidity and mortality.The management of submitral aneurysm includes surgical correction by repair of the aneurysm but it requires careful consideration of surgical resection of diseased paravalvular tissue or diseased mitral valve and correction of mitral regurgitation by preforming mitral valve replacement. The surgical approach reduces the risk of recurrent endocarditis and thromboembolic events, thus reducing the risk of stroke. Prior to surgery, patient should receive prophylactic antibiotics for one to two weeks to control any active infective endocarditis. For us successful treatment of a patient suffering from submitral aneurysm due to infective endocarditis was challenging. We are reporting a case of submitral aneurysm secondary to infective endocarditis along with review of literature on infective endocarditis and submitral aneurysm.

IntroduçãoO aneurisma idiopático do ventrículo esquerdo é uma entidade clínica pouco comum e raramente descrita na literatura que usualmente se origina na região anular sub-aórtica ou sub-mitral, e sua etiologia não isquêmica permanece pouco estabelecida. Esta patologia pode ter diferentes apresentações clínicas desde regurgitação mitral e insuficiência cardíaca, até arritmias ventriculares potencialmente fatais e morte súbita. Embora a grande maioria de casos inicialmente fosse descrita em população de raça negra, se tem descrito também em população caucasiana em menor incidência. Na atualidade, o uso de multi-modalidade de imagens permite uma melhor triagem e caracterização das lesões. A seguir é apresentado caso em uma paciente sem evidência de doença coronariana conhecida, e presença em imagens de ecocardiografia e ressonância magnética cardíaca de lesão aneurismática sub-valvular mitral idiopática com comprometimento funcional da válvula mitral, e na qual se descartaram outras causas etiológicas, sendo feita a correção cirúrgica com uma adequada evolução posterior ao procedimento. O diagnóstico foi confirmado com exame histopatológico, estudo de sorologia e perfil imunológico. Relato do CasoPaciente de sexo feminino, 46 anos, mestiça, casada. Consulta por quadro clínico de 4 meses de evolução consistente com dispneia aos moderados esforços (Classe funcional NYHA II). Refere episódios de dor torácica opressiva ao caminhar sem irradiação típica que cede com o repouso. Nega sintomas congestivos; não há dispneia paroxística noturna nem edemas. Interrogatório sem episódios sincopais e não tem apresentado taquicardia ou palpitações. Antecedentes patológicos de hipotiroidismo diagnosticado em 2011 e neurite óptica nos últimos 6 meses a qual foi tratada com aciclovir pelo serviço de oftalmologia por suspeita de etiologia viral. Não existem antecedentes de doenças reumatológicas nem doenças sistêmicas de relevância. Nega antecedentes toxicológicos ou trauma torácico prévio. No exame físico se encontra em adequadas condições gerais embora com valores tensionais elevados (150/65 mmHg) com sopro sistólico mitral II/VI. Realizou-se eletrocardiograma que evidência necrose inferior antiga e inversão de onda T em derivações II, III e a VF. A paciente foi encaminhada para o serviço de cardiologia não invasiva para realização de ecocardiograma transtorácico revelando hipertrofia ventricular esquerda excêntrica com disfunção sistólica moderada (fração de ejeção estimada em 40-45%), dilatação do ventrículo esquerdo com diâmetro tele-sistólico de 35 mm/m 2 e disfunção diastólica por alteração do relaxamento. Na análise de contratilidade segmentaria se evidenciam distúrbios segmentários da contratilidade segmentar se evidenciam acinesia e adelgaçamento da parede inferior em todos seus segmentos, com um grande saco aneurismático que demonstra evidência de fluxo com Doppler colorido no seu interior (Relação colo/saco 0.4) (Figura 1).
