Ablação com radiofreqüência do flutter atrial tipo I. Importância do bloqueio bidirecional do istmo entre a veia cava inferior e o anel da valva tricúspide

Abstract: Purpose -To determine the clinical importance of a bi-directional line of block demonstration in the inferior vena cava-tricuspid annulus isthmus as an end-point for radiofrequency (RF) atrial flutter (FL) ablation. Methods -Forty consecutive patients (51±11 years) with type I FL were divided in 2 groups: GI (30 patients) anatomic, non-electrophysiologic isthmus Conclusion -Electrophysiologic demonstration of bidirectional line of block in the isthmus is related to longterm success and should be the criterion … Show more

“…The efficacy of CTI ablation in the treatment of atrial flutter was initially demonstrated using 4-mm-tip electrode catheters [3][4][5][6][7] . However, bi-directional block of the CTI required an increased number of RF pulses, and that could not be achieved in 5 to15% of patients [3][4][5][6][7] .…”

“…However, bi-directional block of the CTI required an increased number of RF pulses, and that could not be achieved in 5 to15% of patients [3][4][5][6][7] . In addition, recurrence rate was 10 to 25% [3][4][5][6][7] . Those results can be explained by the difficulty in obtaining a continuous, transmural lesion to guarantee a bi-directional block of the CTI with 4-mm-tip catheters [3][4][5][6][7] .…”

“…Patients who primarily present AFL usually also present AF and vice versa. RF ablation is known to be the treatment of choice for recurrent CTI-AFL thanks to its high success rate and low risk of complications 21,22 . However, AF episodes are described in 25% to 36% of the patients undergoing ablation of CTI-AFL [5][6][7][8][9] .…”

“…A ablação com radiofreqüência (RF) é um tratamento efetivo para o controle do flutter atrial (6)(7)(8)(9)(10)(11)(12) . O Ist-CT tem sido o alvo da intervenção pelo fato de ser a área mais estreita do circuito, com limites definidos, o que facilita a realização do procedimento (13,14) .…”
