Severe coinfection of melioidosis and dengue fever in northeastern Brazil: first case report

Abstract: This report focuses on a fatality involving severe dengue fever and melioidosis in a 28-year-old truck driver residing in Pacoti in northeastern Brazil. He exhibited long-term respiratory symptoms (48 days) and went through a wide-ranging clinical investigation at three hospitals, after initial clinical diagnoses of pneumonia, visceral leishmaniasis, tuberculosis, and fungal sepsis. After death, Burkholderia pseudomallei was isolated in a culture of ascitic fluid. Dengue virus type 1 was detected by polymerase… Show more

“…Several studies have demonstrated that it is difficult to differentially diagnose dengue from other pathologies (e.g., hantavirus, rubella, hepatitis, influenza A infection, or melioidosis), which may account for some of the cases of fever symptoms that were not diagnosed as leptospirosis in this study 17 , 18 , 25 . Those studies' findings highlight the importance of using surveillance protocols for fever syndromes, and the implementation of these systems in Brazil may facilitate the early detection of new diseases with similar clinical characteristics, such as chikungunya fever.…”

“…It has been unusual to detect B. pseudomallei with other pathogens in patients. Dengue and melioidosis coinfection was reported in Northeastern Brazil [ 10 ]; A case of leptospirosis and melioidosis co-infection occurred in Malaysia [ 11 ]. Melioidosis coinfection with other disease e.g.…”

“…Four siblings acquired acute infection after recreational exposure at the town dam and three of them died within a week [ 7 , 8 , 9 , 10 ]. By 2017, 30 cases had been diagnosed in Ceará [ 2 , 11 , 15 , 16 , 17 , 18 , 23 ]. Characterisation of clinical and environmental B. pseudomallei from Ceará showed that considerable genetic diversity is present [ 30 ].…”
