Disseminated nocardiosis with psoas abscess in a patient with AIDS: first reported case

Abstract: SUMMARYPsoas muscle abscess is an uncommon infection that have been diagnosed increasingly in the last years. We present a case of a patient with advanced human immunodeficiency virus infection who developed a disseminated infection due to Nocardia asteroides sensu stricto type VI with psoas abscess. To our knowledge no other cases of Nocardia psoas abscess in the setting of HIV infection have been reported in the literature.

An extensive literature review yielded only 20 cases, with 9 abscesses in the thigh 4,5,21-24 , 6 in the iliopsoas muscle 5,7,9,21,25,26 , 2 in the calf muscles 27 , 1 in the gluteal muscles 5 , 1 in the lower abdominal wall musculature 21 , and 1 in the deltoid muscle 21 . Although some cases have been reported in immunocompetent patients due to trauma or iatrogenic causes, the predominant predisposing factor is immunosuppresion induced by corticosteroid therapy, 2,6 heart transplantation, 7 chemotherapy 8 and AIDS 9 .
