Background: Republic of Azerbaijan and Iran are two neighboring countries with high amount of travelling between them, different rates of TB and its resistance. These relationships could complicate the controlling of tuberculosis programs. Objective: The study was conducted to determine the prevalence of tuberculosis (TB) resistance and its risk factors in the two geographically co-related regions. Method: A total of 119Mycobacterium tuberculosis isolates from the patients of the target regions (Azerbaijan Rep. and Tabriz, Iran) were examined at the Central Tuberculosis Laboratory in Tabriz, Iran. The cultures and drug susceptibility tests was performed on Lowenstein-Jensen. All the isolates were categorized by MIRU-VNTR molecular method into clustered and un-clustered groups. The clustering as well as demographic data were analyzed to determine the risk factors contributing to TB resistance. The categorical data about the TB resistance were compared using a chi square test. Results: 27.8 percent of isolates were resistant at least to one of the 1st line of anti-tuberculosis drugs. The prevalence of any drug resistance and MDR were 64.6 and 17.9 percent respectively for Azari isolates, where the rate of resistance to rifampin and isoniazid was higher than streptomycin and ethambutol. The corresponding figures for the isolates from Tabriz were 16.9 and 3.3 percent respectively. Conclusions: The results of this comparative and cross-sectional study showed statistically significant differences in TB resistance between the isolates from the patients of Azerbaijan Rep. and Iran. According to the results, the rate of resistance to RMP, INH, and MDR was higher in Azerbaijan; consequently, nationality could be regarded as a risk factor of MDR, resistance to RMP and INH but not to SM and ETB. Резюме. Исследовано состояние иммунитета у больных прогрессирующим ФКТ с различной выраженностью деструктивных изменений в легких. Установлено, что признаком увеличения деструкции является нарастание числа специфических цитотоксических лимфоцитов, NK-, NKT-, CD95клеток и активированных T-лимфоцитов при значительном снижении количества Т-хелперов, В-лимфоцитов, CD25-клеток, наряду с угнетением антигенспецифического клеточного ответа и функциональной несостоятельностью Т-хелперов и цитотоксических лимфоцитов. Увеличение тяжести процесса согласуется с активацией В-звена и ослаблением фагоцитарной функции нейтрофилов. Выявлены существенные отличия в спонтанной и индуцированной продукции IFNγ, IL-2 и IL-8 у различных групп больных ФКТ. Выявленные нарушения в иммунном и цитокиновом статусе у определенной части больных прогрессирующим ФКТ указывают на истощение возможностей иммунной системы. На основании полученных данных установлены критерии, определяющие тяжесть иммунопатологических проявлений у больных прогрессирующим ФКТ и прогностически значимые при определении тенденций развития специфического процесса и целесообразности проведения иммунокоррекции.

Syzygium aromaticum is used in traditional and modern medicine for its various and outstanding pharmacological properties. Here, we studied the chemical composition of hexane extract and non‐polar fractions (NPF) obtained from the maceration and fractionation of clove buds, in order to evaluate their in vitro antimycobacterial activity, as well as their contribution against efflux pump (EP) resistance through molecular docking experiments. The gas chromatography‐mass spectrometry (GC–MS) analysis of the volatile profiles revealed the presence of eugenol, followed by eugenyl acetate, and β‐caryophyllene as common major compounds. According to Resazurin microtiter assay (REMA), Mycobacterium tuberculosis H37Rv strain was sensitive to all volatile samples at concentration range between 10 and 100 μg/mL. The NPF of ethanol extract was the best inhibitor with a MIC=10 μg/mL. The in silico study revealed a strong binding affinity between eugenol and Mmr EP protein (−8.1 Kcal/mol), involving two binding modes of hydrogen bond and π‐alkyl interactions. The non‐polarity character of clove volatile constituents, and their potential additive or synergistic effects could be responsible for the antimycobacterial activity. In addition, these findings suggest the benefic effect of eugenol in the management of mycobacterium drug resistance, whether as potential inhibitor of Mmr drug EP, or modulator during combination therapy.
