Papular-purpuric "gloves and socks" syndrome due to parvovirus B19: report of a case with unusual features

Abstract: SUMMARYWe present a case of papular-purpuric "gloves and socks" syndrome (PPGSS) in an adult male with acute parvovirus B19 infection. The patient displayed the classical features of fever, oral lesions, and purpura on hands and feet, but the purpuric lesions on the feet evolved to superficial skin necrosis, a feature not previously described in this syndrome. We believe this is the first reported case of PPGSS occurring in Brazil.

“…17 Bullous expression of the skin lesions has so far been reported in only three patients, all of whom were more than 35 years old. 10,12,16 Both extraacral localization of skin lesions and bullae were observed in our patient. The various oral manifestations, which have been described for more than half of PPGSS patients, 4 include petechiae, erythema and swelling, vesicles, aphthous ulcers and Koplik spots on the hard and soft palate and the labial and buccal mucosa.…”

“…The exanthem often progresses rapidly from generalized erythema to a more petechial or papular/purpuric appearance, rarely with associated bullae. 6,39,40 The affected extremities are often pruritic and may be painful. Oral lesions frequently accompany the petechial/papular/purpuric transition and may manifest as petechiae, vesicles, pustules, aphthous ulcers, pharyngeal erythema, angular cheilitis, and Koplik spots.…”
