RESUMOEste estudo avaliou fatores individuais, rela vos ao trabalho e organizacionais relacionados à adesão às precauções-padrão por profissionais de enfermagem que atuam em terapia intensiva. Estudo de corte transversal, realizado com 178 profi ssionais em um hospital de grande porte. Para a coleta de dados foram utilizadas escalas psicométricas do po Likert. Na Escala de Adesão às precauções-padrão obteve-se um escore de 4,45 (DP=0,27) classifi cado como intermediário. Houve correlação quando comparada com fatores individuais para Escala de Personalidade de Risco (r=-0,169; p=0,024) e fatores relativos ao trabalho com a Escala de Obstáculos para seguir as precauções-padrão (r=-0,359; p=0,000). A adesão às precauções-padrão entre profi ssionais de enfermagem foi intermediária. Fatores individuais e rela vos ao trabalho infl uenciaram a adesão às precauções-padrão. Background Standard Precautions (SP) are designed to limit blood-borne pathogen exposures among healthcare workers (HCW) and healthcare associated infections. SP adherence is globally suboptimal; yet reasons are underexplored. This study aim was to explore the relationships among safety climate factors and SP adherence by HCW in hospitals using newly developed survey and observational tools. Methods Cross-sectional data from 11 units in five hospitals were collected between March and September 2015. A patient safety and standard precaution survey was administered to nurses and pooled with data from observations of HCW-patient interactions using defined SP indications. Descriptive statistics of distributions, frequencies, and Pearson correlation coefficients were calculated using SAS to determine the unit-level relationships among dimensions of the patient safety climate and unit percentage of SP adherence (p < .05). Results 540 HCW-patient encounters with 1,713 SP indications and 140 surveys and were collected. Although most nurses (94%) reported “always” or “often” adhering to SP and generally reported positive scores on unit safety climate, observed SP adherence was 62% (unit range 31–80%). Only 30% of nurses rated staffing positively, and this was inversely related to observed SP adherence. Conclusions Adherence to the full complement of observed SP behaviors by HCW of all types was suboptimal. The relationship between safety climate, particularly staffing, and adherence to SP warrants further testing.

Background: Standard precautions (SPs) serve as the first line of defense against exposure to blood and body fluids. The objective of this study was to explore the adherence of nurses to SPs and to identify factors influencing adherence to SPs. Methods: This study was an exploratory cross-sectional survey. A total of 339 nurses from 9 general hospitals and 3 tertiary hospitals located in 3 South Korean cities were selected. Hierarchical regression was used to examine the effects of sociodemographic, individual, and institutional factors. Results: A higher, or positive, attitude was the strongest influencing factor in adherence to SPs in the final model, followed by administrative support, hospital types, and safety climate, in descending order. These 4 variables accounted for 26.0% of the variance in adherence to SPs. Conclusions: The attitudes of nurses toward SPs is important for increasing the adherence to SP best practices. The adherence of nurses to SPs will improve if safe environments are created in different hospital types and if managerial support and administrative efforts are supportive and sustained.
