OBJECTIVE:To identify predictive indicators of institutionalization of the elderly. METHODS:A case-control study was carried out with 991 elderly individuals in the city of Pelotas, (Southern Brazil), from 2007 to 2008. The cases of institutionalized elderly adults (n = 393) were detected using a census of all long-stay institutions for the elderly in the city. The population controls (n = 598) were randomly selected using a comprehensive health survey. Pearson's chi-square test and linear trends were used to compare groups in the crude analysis; and the binary logistic regression model of the adjusted analysis, with the effects expressed as odds ratios. RESULTS:Institutionalization was more frequent in females (OR = 1.96, 95%CI 1.31, 2.95). Elderly with advanced age (OR = 3.23 and OR = 9.56 for age groups 70-79 and ≥ 80 years, respectively), those who lived without a partner (single, divorced or widowed), and those who had no formal schooling or had a functional disability preventing them from performing basic activities for daily living were more likely to be institutionalized. An inverse trend between the incidence of elderly institutionalization and the level of physical activity was observed, where somewhat active and inactive subjects were more likely to be institutionalized (OR = 1.71 and OR = 4.73, respectively). CONCLUSIONS:Of the factors examined, age ≥ 80 years, living without a partner and being physically inactive were the indicators most strongly associated with institutionalization. The encouragement of informal care through cultural and educational activities focused on the role of the family in caring for the elderly can prevent the institutionalization of these individuals. OBJETIVO: Traçar o perfil dos idosos que foram a óbito por queda no Rio Grande do Sul no período de 2006 a 2011. MÉTODOS: Foram analisados 2.126 óbitos por queda em idosos no RS de 2006 a 2011, registrados no Sistema de Informações sobre Mortalidade. As análises estatísticas foram realizadas utilizando-se o aplicativo computacional PASW 17.0. RESULTADOS: A chance de óbitos por queda nos idosos é significativamente maior para o gênero feminino, para a faixa etária acima de 69 anos, para idosos com cor de pele branca, para viúvos ou solteiros. Houve acréscimo de 41,8% nos coeficientes de mortalidade específicos por queda no período do estudo, o maior coeficiente ocorrendo em 2011 31,56 óbitos por queda a cada 100 mil idosos, maior para o gênero feminino e para idade de 80 anos ou mais. CONCLUSÃO: Constatou-se que o coeficiente de mortalidade por queda aumentou de 2006 para 2011 naquele estado, sendo mais elevado para os idosos de 80 anos ou mais, resultados de relevância para o desenvolvimento de políticas públicas para o idoso.

Objective: To analyze the influence of changes in function/cognition and the presence of depression in institutionalized elderly who have suffered falls, in order to prevent these accidents. Methods: Quantitative exploratory and descriptive research, conducted with 30 elderly individuals in Rio Grande do Sul (Brazil), using five data collection instruments. The analysis was conducted using SPSS 13.0. Results: We found that: the majority were women; had low levels of education; the risk of falls increased with age; older people with less time in an institution had fallen more. We identified the influence of medication use on falls. In relation to functional capacity, the elderly who had fallen, had independence for activities of daily living; of 20 elderly with scores suggestive of cognitive impairment, ten suffered falls. There was no significant influence between depression and falls in our investigation. Conclusion: Results show the importance of determining the influence of functional / cognitive changes and the presence of depression, and in episodes of falls in institutionalized elderly. Em relação à capacidade funcional, os idosos que caíram, apresentaram independência para as atividades de vida diária; dos 20 idosos com escore sugestivo para deficit cognitivo, dez sofreram quedas. Não houve influência significativa entre depressão e quedas nos investigados. Conclusão: Verificou-se a importância de determinar-se a influência de alterações funcionais/cognitivas e a presença de depressão, em episódios de quedas nos idosos institucionalizados.
