Espondilite anquilosante e anestesia

Abstract: Patients with chronic diseases of the spine represent specific challenges to the anesthesiologist. Handling of the airways and the access to the neuroaxis can be difficult. Most anesthesiologists prefer to use general anesthesia, avoiding the neuroaxis, in those patients, despite the presence of difficult airways. The degree of spine involvement will determine how difficult the tracheal intubation might be. Special care should be taken to avoid excessive manipulation of the neck, which could cause trauma to th… Show more

“…[ 3 ] The progressive involvement of the cervical spine with reduced to absent neck movements, flexion deformities and ankylosis of the temporomandibular joints with reduced mouth opening; makes airway accessibility difficult in AS. [ 1 4 ] Therefore, it becomes imperative to determine the dose and volume of local anaesthetic administered during SAB. The clinical judgement, expert opinion and the limited evidence available justify the use of SAB in infra-umbilical extraperitoneal, perineal and lower limb surgeries.…”

“…The airway and axial skeleton involvement in ankylosing spondylitis (AS) poses unique challenges to the anaesthesiologists. [ 1 ] The spinal manifestations comprise of spondylitis, diskitis, arthritis and enthesitis. These lead to progressive stiffness, ossification of the axial ligaments [ Figure 1 ], obliteration of the intervertebral spaces and syndesmophytes formation; hindering access to the central neuraxial space.…”

“…No caso da necessidade de anestesia geral, é difícil caso a mobilidade cervical esteja comprometida, ou então com a abertura bucal limitada se houver alguma restrição na articulação temporomandibular. Além disso, o envolvimento extra-articular também pode complicar a administração da anestesia geral (Oliveira, 2007;Hoffman et al 2012).…”
