Noise and hypertension: study of their association among female teachers

Purpose: investigate the association of noise exposure with the occurrence of systemic arterial hypertension (SAH) among public school female teachers. Methods: a cross-sectional, descriptive and observational study was conducted. A questionnaire elaborated by the researchers was applied to a random sample of 133 female teachers. Noise exposure dosimetry was used among the sample subjects. The non-conditional logistic regression technique was applied to identify significant variables and control confounding variables for the occurrence of SAH. Results: the prevalence of hypertension found, in teachers, in this study, was 17.3%. However, the association between occupational noise exposure and the occurrence of hypertension was not verified. It was observed that smoking is a very relevant risk factor for hypertension, while the physical effort performed at work appeared as a protection factor. Conclusion: although the variable "noise exposure dose" was not significant in the final model, it was found that the exposure values were above the limits, justifying the adoption of preventive measures.

Smoki ng is related to 30% of cancer deaths. It is a risk factor for respiratory tract, esophagus, stomach, pancreas, uterine cervix, kidney and bladder carcinomas. Nicotine induces tolerance and addiction by acting on the central dopaminergic pathways, thus leading to pleasure and reward sensations within the limbic system. It stimulates the central nervous system (CNS), enhances alertness and reduces the appetite. A 50% reduction of nicotine consumption may trigger withdrawal symptoms in addicted individuals: anxiety, anger, sleep disorders, hunger, cognitive dysfunction and cigarette craving. Medical advice is the cornerstone of smoking cessation. Pharmacotherapy of nicotine addiction comprises first-line (bupropion and nicotine replacement therapy) and second-line (clonidine and nortriptyline) drugs. Bupropion is a non-tricyclic antidepressant that inhibits dopamine uptake, whose contraindications are: epilepsy, eating disorders, uncontrolled hypertension, recent alcohol abstinence and current therapy with MAO inhibitors. Nicotine replacement therapy can be done with patches or gums. Counseling groups and behavioral interventions are efficacious. The effects of acupuncture on smoking cessation are not fully elucidated. Prompt smoking cessation or gradual reduction strategies have similar success rates.

RESUMO Objetivo: Foi investigado o impacto da abstinência do tabaco na pressão arterial (PA) e frequência cardíaca (FC) na resposta a uma sessão de exercício aeróbio. Materiais e Métodos: Cinco fumantes (23,6±1,9 anos), previamente sedentários, realizaram três sessões de exercício em esteira com duração de 30 minutos e intensidade entre 60 e 70% da FCmax, mantendo abstinência de seis horas (ABS), fumando um cigarro cinco minutos antes do exercício (FIM) ou 1 hora após fumar um cigarro (F1H). Um grupo de não fumantes (23,2±0,4 anos) realizou uma sessão controle (CON). FC e PA foram monitoradas antes, durante e após as sessões de exercício. Os dados foram apresentados em média e erropadrão. Foi utilizado o teste ANOVA, com post-hoc de Tukey para comparar os procedimentos. Resultados: FC e PA se mostraram similares durante e imediatamente ao final do exercício para todas as situações (p> 0,05). Entretanto, 30 minutos após o exercício, a PA diastólica mostrou-se significativamente aumentada em FIM (77,6±3 mmHg) comparado com controle (p<0,05), enquanto que em ABS, F1H e CON ocorreu redução (68,5±3, 71,6±4 e 66,0±6 mmHg respectivamente), sem diferença significativa. Conclusão: a abstinência do tabagismo não altera as respostas de FC e PA em fumantes durante o exercício, mas reduz os benefícios agudos do exercício sobre a redução da PA pós-exercício. SUMMARY Introduction:It was investigated the impact of tobacco abstinence on blood pressure (BP) and heart rate (HR) in response to an aerobic exercise session. Materials and Methods: Five smokers (23.6±1.9 years), previously sedentary, performed three treadmill exercise sessions lasting 30 minutes with intensity between 60 and 70% HRmax, maintaining abstinence from six hours (ABS), smoking a cigarette five minutes before exercising (FIM) or 1 hour after smoking a cigarette (F1H). A group of nonsmokers (23.2 ± 0.4 years) performed a control session (CON). HR and BP were monitored before, during and after exercise sessions. The data were presented as mean and standard error. ANOVA test was used, with Tukey post-hoc test to compare procedures. Results: HR and BP were similar during and immediately at the end of the exercise for all situations (p> 0.05). However, 30 minutes after exercise, the diastolic BP was found to be increased by FIM (77.6±3 mmHg) compared with control (p <0.05), while in ABS, F1H and CON reduction occurred (68.5 ± 3, 71.6 ± 4 and 66.0 ± 6 mmHg respectively) with no significant difference. Conclusion: smoking abstinence does not alter the responses of HR and BP in smokers during exercise, but reduces the benefits of acute exercise on reducing post-exercise BP. DESCRIPTORS
