Objectives: To evaluate the results of the comprehensive geriatric assessment (CGA) before allogeneic hematopoietic stem cell (HSCT) transplantation in patients aged 60 years and over. Methods: We evaluated all consecutive patients undergoing CGA before HSCT between September 2011 and July 2018 in a private hospital in Brazil. We also evaluated the Hematopoietic Cell Transplantation-Specific Comorbidity Index (HCT-SCI) and the Disease Risk Index (DRI). Results: During the study period, 61 patients were referred for transplant evaluation. After exclusions, we analyzed 40 patients, with a mean age of 67.6 years (60-76). The CGA detected vulnerability and frailty in 43% and 18.9% respectively according to the Fried Frailty Phenotype score; limitations across the domain of function and disability with handgrip test alterations in 65.8%. However, 36 (90%) were independent for instrumental activities of daily living (IADL). Cognitive and depression domain have shown abnormal with the clock test in 44.4%, and loss of memory complains in 37.5%. But the mini-mental test was normal in 89%. Geriatric Depression Scale (GDS) was normal in 82.5%. 30% were considered at risk for malnutrition. Half of the patients (50%) had a high Hematopoietic Cell Transplantation Comorbidity Index (HCT-CI) score. 32.5% needed ICU admission. The overall survival and non-relapse mortality at 2 years were 41.8% and 38.7% respectively. Conclusion: The CGA was feasible in detecting the patients' vulnerabilities in our population. More studies, multicentric and with a larger number of patients, are needed to evaluate the role of CGA in this context of allo-HCT in our population.

This study aimed to analyze the functional capacity of elderly registered in Basic Health Units of Guarapuava-PR, Brazil. The subjects were 359 elderly, interviewed from January to April 2010, using Section I and IV of the questionnaire Brazil Old Age Schedule. The results showed that 89.9% had some degree of dependency, of which 70.8% light and 19.2% moderate or severe dependence. Women have a higher degree of mild (70.5%) and moderate and severe dependence (62.3%). Factors associated with the degree of dependence among the elderly living in Guarapuava were age, schooling, living arrangements, employment, place of residence, marital status and family income. It is concluded that most of the elderly in the community is affected by limitations and live with some kind of dependence to perform basic activities of daily living. ReSUmÉNEl estudio objetivó analizar la capacidad funcional de los ancianos inscritos en Unidades Básicas de Salud de la Guarapuava-PR, Brasil. Participaran 359 personas mayores, entrevistadas entre enero y abril de 2010, con la Sección I y IV del cuestionario Brazil Old Age Schedule. Los resultados mostraron que 89,9% tenían algún grado de dependencia, de los cuales 70,8% tenían una dependencia leve y 19,2% moderada o grave. Las mujeres tenían un mayor grado de dependencia leve (70,5%) y moderada y grave (62,3%). Los factores asociados con el grado de dependencia entre los ancianos que viven en Guarapuava fueron la edad, la escolaridad, arreglos de vivienda, empleo, lugar de residencia, estado civil y el ingreso familiar. Se concluye que la mayoría de las personas mayores en la comunidad se ve afectada por las limitaciones y vivir con algún tipo de adicción para realizar actividades básicas de la vida diaria. -Functional fitness loss during aging may compromise the quality of life and independence of older subjects. It is important to evaluate and diagnose the functional fitness of the elderly population. This study proposed normative functional fitness scores for a sample of elderly women from southern Brazil and compared values to their counterparts in the US, Extremadura (Spain), Taiwan (China) and Spain. The study sample consisted of 1,783 older women aged 60.0 to 84.9 years (mean 68.7 years; standard deviation 6.3 years) who performed the proposed motor tests of the "Senior Fitness Test" for functional fitness in older women. The percentile values specific to each age group were calculated based on the seven functional fitness components: body mass index, 6-minute walk, arm curl, 30-s chair stand, chair sit-and-reach, backscratch, and 8-feet up-and-go. The non-parametric binomial test compared the 50th percentile value of Brazilian older women with those from other countries. Older women´s performance in the functional capacity tests decreased across age groups. The mean BMI varied among age groups from 29.11 to 26.76 kg/m 2 , 6-minute walk from 572.94 to 486.95 m, arm curl from 17.51 to 15.11 repetitions, 30-schair stand from 15.62 to 14.30 repetitions, chair sit-and-reach from 1.01 to -0.47 cm, back scratch from -4.92 to -10.52 cm and 8-feet up-and-go from 5.96 to 6.83 sec. Functional fitness scores among older women in different countries differed significantly. However, the direction and magnitude of differences were specific to the functional fitness component. Significant differences were observed in the normative scores, suggesting that the use of international normative scores in Brazilian older women may underestimate or overestimate potential functional limitations.
