Small-fiber polyneuropathy in leprosy without skin changes: study of 17 cases

Abstract: -Leprosy is one of the most common diseases of the peripheral nerves. In some cases there is only neural involvement without skin changes (neuritic form). The neuropathy has often a distal stocking and glove distribution with thermal and pinprick anesthesia and preservation of proprioception. There is no weakness, the tendon reflexes may be preserved and sometimes the nerves are thickened. We reported 17 patients with a predominantly small-fiber polyneuropathy due to leprosy. All patients had distal temperatur… Show more

“…Difficulties for the diagnosis can occur in some pure ulnar palsy cases. Only a dorsal ulnar nerve superficial branch biopsy confirms leprosy diagnosis 4 . Sonography and electrophysiology were complementary for identifying ulnar nerve neuropathy in 21 consecutive Brazilian patients with leprosy, with clinical symptoms as the reference standard 8 .…”

“…leprae is morphologically indistinguishable from M. tuberculosis. It damages both myelinated and unmyelinated nerve fibers followed by replacement of functional tissue by conjunctive tissue -fibrosis 4 . The pathogenesis of nerve injury can be summarized with the localization of the M. leprae to nerve, Schwan cell infection and self responses, demyelination and axonal atrophy, accompanying with oedema and characteristic inflammation.…”

“…In all forms of leprosy occurs the presence of the bacilli in the nerves, which is housed in the Schwann cells in both myelinated and demyelinated axons. These not inflammatory phenomena promote early demyelination, occurring even before the body perceives the presence of the bacillus, being called parasitation (14).…”

“…In most cases, these neuropathies usually follow the skin manifestations although the neuropathy may be the only manifestation of the disease in the pure neuritic leprosy (PNL) 3 . Out of 19 PNL cases we had studied 4 , 79% presented mononeuropathy, 10.5% multiple mononeuropathy and 10.5% polyneuropathy.…”
