The Rolex sign - first manifestation of Parkinson's disease: case report

Abstract: -We describe a patient whose initial manifestation of Parkinson's disease was a malfunctioning of a self-winding wristwatch secondary to bradykinesia of his left arm. Andrade and Ferraz reported this sign in 1996, suggesting that it be called the Rolex sign. The clinical manifestation of Parkinson's disease is frequently subtle, making an early diagnosis a difficult task. The patients' friends, who notice a lack of mobility, are the first ones to draw their attention to the problem. Of the cardinal signs of th… Show more

“…Common nonmotor symptoms at PD onset (or even preceding PD motor symptoms) include fatigue,1, 3 decreased sense of smell,8 depression/anxiety,9 rapid eye movement sleep behavior disorder,10 and neck/back pain 1. Unusual presenting motor symptoms of PD include the “Rolex sign,” where a PD patient wearing a self‐winding wristwatch notices it does not wind secondary to reduced arm swing,11, 12 bruxism,13 kinesigenic foot dystonia,14 paroxysmal exercise‐induced dystonia,15, 16 or even recurrent dyspneic attacks due to laryngeal dystonia 17. PD should be included in the differential diagnosis of patients presenting with throat clicking.…”
