RESUMO -Apresentamos dois casos de histoplasmose em que o acometimento do sistema nervoso central foi a única manifestação clínica da doença. Revisão da literatura permitiu confirmar a raridade dessa forma de patologia, em particular em crianças, sendo o segundo caso aqui apresentado o de mais baixa idade na literatura nacional. São feitos comentários sobre a variedade das apresentações clínicas e os principais diagnósticos diferenciais da doença. São discutidos os resultados laboratoriais, tanto em relação às alterações liquóricas quanto aos resultados sorológicos. Finalmente, é analisada a terapêutica dessa forma de infecção fúngica tanto em relação aos medicamentos utilizáveis quanto à duração prolongada aconselhável para o tratamento específico.PALAVRAS-CHAVE: histoplasmose cerebral, meningite crônica, terapêutica antifúngica. A clinical review confirmed the infrequency of this form of the disease, overall in childhood, being one of these cases the youngest in Brazilian reports. Comments about the diversity of clinical presentation and main differential diagnosis are presented. We analyze the serologic and cerebrospinal fluid results and, finally, discuss the drugs and duration of treatment.KEY WORDS: cerebral histoplasmosis, chronic meningitis, antifungal therapy. A infecção pelo Histoplasma capsulatum é relativamente comum em nosso meio. Lacaz e col.1 , empregando o teste cutâneo da histoplasmina, verificaram positividade de 16,0% no Rio de Janeiro, 19,0% em São Paulo, 19,2% em Goiás e 27,5% no Pará. A maioria dessas infecções, no entanto, é oligo ou assintomática, sendo os casos com manifestação clí-nica franca uma pequena minoria (menos que 5% nas exposições de baixo nível, porém mais comum após exposições mais intensas 2 ). As formas mais frequentemente descritas da histoplasmose são basicamente de dois tipos: 1.Doença progressiva grave, em geral disseminada, em indivíduos com comprometimento da imunidade. Patients and methodsChildren admitted to the paediatric warI aged 1 month to 16 years with bacterial meningitis in 1988-93, were included in this study. A diagnosis of bacterial meningitis was made when either bacteria were cultured from CSF using standard methods, or there were clinical signs of meningococcal septicaemia (with purpura), and CSF leucocytosis in the absence of a positive culture. In each case information was obtained with regard to age of the child, results of latex agglutination, Gram staining, and culture, and whether antibiotic treatment had started before obtaining the CSF sample.The latex agglutination test kit (Wellcogen) is used for the qualitative detection of specific polysaccharide surface antigens from streptococcus group B; H influenzae type b; S pneumoniae; Escherichia coli KI; N meningitidis groups A, C, Y, W135 and group B. It may be used to detect antigen in various body fluids but in this study only CSF results were considered. ResultsSeventy two children were admitted with bacterial meningitis over the five year study period, but latex agglutination test results were performed in only 48 of these cases. Of these 48 children, 39 had a positive CSF culture result and another nine were included in the study as they were considered to have meningococcal meningitis, with CSF leucocytosis, and clinical features of meningitis plus a purpuric rash, in the absence of a positive CSF culture result.In the 48 children considered in this study, the aetiology was confirmed in 28 cases (58%) by latex agglutination and in 30 cases (63%) by Gram staining (see
