Hepatitis B status in hemodialysis patients

Abstract: Despite the benefit of dosing of anti-HBc and anti-HBs before admission to dialysis, economic constraints have reduced the screening to only HBsAg. Since occult HBV infection has already been demonstrated in hemodialysis patients, the measure of anti-HBc should be encouraged.

“…In Brazil, prevalence of HBsAg of 0, 2.4, 7, 10, 34.1% were reported in hemodialysis center in Southeast, Central, Northeast, South and Midwest regions of Brazil [78][79][80][81][82]. HBsAg prevalence of 4.5, 25, 1.4% was found in hemodialysis patients in Peru, Venezuela, Cuba [59,67].…”

“…Patients receiving hemodialysis (HD) are at an increased risk of hepatitis B infection for a variety of reasons including potential exposure due to improper disinfection of fomites (e.g., chair, equipment, dialysis machine), frequent diagnostic and therapeutic procedures, immune dysfunction, and reduced rates of vaccination response . Hepatitis vaccination has a 95% to 100% response rate in the general population compared to lower response rates reported in HD patients ranging from 55% to 88% .…”

“…Patients receiving hemodialysis (HD) are at an increased risk of hepatitis B infection for a variety of reasons including potential exposure due to improper disinfection of fomites (e.g., chair, equipment, dialysis machine), frequent diagnostic and therapeutic procedures, immune dysfunction, and reduced rates of vaccination response. [1][2][3] Hepatitis vaccination has a 95% to 100% response rate in the general population 4 compared to lower response rates reported in HD patients ranging from 55% to 88%. [5][6][7][8][9][10][11] Due to lower response rates in HD patients, the current recommendation is to administer a higher dose regimen of 40 μg given either as a 3-dose regimen (Recombivax-HB ® , Merck Frosst, Quebec, CA) or 4-dose regimen (Engerix B ® , GlaxoSmithKline, Ontario, CA) ( Table 1).…”

“…The reasons for high susceptibility to HBV among HD patients include invasive medical HD procedures, increased exposure to blood product transfusions, shared machines and equipment, and high prevalence of HBV among HD patients [7,10]. Prominent fibrosis, silent clinical presentations, and atypical serologic evolution are prevalent among dialysistreated HB patients [11]. A comprehensive systematic review of the HBV infection in Iranian HD patients showed that the HBV rate varied from 0 to 9.75% among these patients across different provinces [12].…”
