Nonsurgical treatment for a severe anterior and lateral open bite and multiple congenitally missing teeth: a case report with 4-year follow-up

Abstract: This case report describes the treatment of a severe anterior and lateral open bite combined with multiple congenitally missing teeth. A 10-year-old girl presented with an open gonial angle, absence of lip sealing, and soft tissue pogonion retrusion. She had an open bite of 8.5 mm, agenesis of the upper right and left lateral incisors and the upper left first premolar, and transverse maxillary deficiency. “…The treatment of hypodontia varies according to its complexity, being more critical in young and growing patients, impairing both the psychological aspect and facial development of these individuals, requiring multidisciplinary treatment. 2 , 15 , 16 Even though the agenesis of mandibular incisors is not among the most frequent manifestations of hypodontia, 1 , 8 it represents a complex clinical challenge, since in most cases there is loss of the deciduous tooth, impairing the function and occlusal balance, often leading to deep bite, residual overjet, mandibular midline deviation and positive tooth-size discrepancy due to the tooth absence. 10 …”

“… 5 - 8 Orthognathic surgery is indicated for adult patients with severe skeletal open bite and unesthetic facial proportions. 9 , 10 For patients with less severe skeletal open bite and those who do not wish to undergo surgery, nonsurgical fixed appliance therapy with intermaxillary vertical elastics and, occasionally, extraction has been a viable alternative. However, this treatment often results in a poor facial esthetics and significant side effects such as root resorption.…”
